POSITION SUMMARY: Under the direct supervision of the Clinical Supervisor, the Peer Support Worker will provide practical support services to individuals, residing within the Harlem and Casa Esperanza Shelters, with histories of co-occurring mental health and substance use disorders (SUD). The Peer Support Worker will deliver on-site person-centered recovery support and harm reduction services to promote recovery recover services including treatment, reduce risk of overdose, prevent the spread of infectious disease, and reduce stigma. POSITION DESCRIPTION: Universal harm reduction education for all clients at intake. Assist with making referrals to harm reduction, substance use, primary care, mental health care, pain management or other as needed. Support linkage to care, return to care, retention in care. Accompaniment to appointments and supporting access to tele-health services. Treatment adherence support (reminders, check-ins, etc.). Support discharge and transition planning. Documentation and reporting of services delivered. Participate in Community of Practice and Learning of others in this role citywide (facilitated by DHS and OASAS). Co-facilitate groups (HHRP, ADL’s, Recovery Peer Support, & others as directed) Adhere to confidential policy and HIPPAA protocol as related to client’s information/interactions. Perform all other duties assigned by supervisor or manager. JOB QUALIFICATIONS: High School Diploma or equivalent. Certified Recovery Peer Advocate (CPRA) certification preferred. If the applicant does not hold this certification, then they must obtain within one-year of employment. Prior experience working with the target population. Ability and willingness to share life experience in a manner appropriate to the consumers served. Good interpersonal skills, enthusiastic about recovery. Ability to speak, read and write the English language, additional languages preferred (Spanish) Proficiency with computer operation (Microsoft Word, Excel, Electronic Health Record and Outlook programs) Must be able to work a flexible schedule including evenings and weekends JOB SETTING: Shelter setting. Must possess sight, hearing and digital senses or prosthetics that ensure the completion of the essential functions of the Clinical Supervisor. Occasionally must lift a maximum of 10 lbs.
