Solcere is a cutting-edge cognitive health clinic dedicated to transforming the lives of individuals and families facing memory loss, dementia, and cognitive decline. We are seeking a motivated, enthusiastic, and compassionate Nurse Practitioner (NP) to join our team and lead and grow our GUIDE Program—an innovative model that integrates clinical care with essential community resources for patients and caregivers. Key Responsibilities: Provide patient-centered clinical care within the GUIDE program framework. Ramp up and scale the GUIDE program through outreach, education, and marketing strategies that engage local providers, patients, and caregivers. Take a lead role in care navigation, guiding patients and families through medical care, community resources, and supportive services. Build and maintain community partnerships with organizations offering respite care, transportation, meals programs, home health, and private duty nursing. Connect with Area Agencies on Aging and other local service providers to expand supportive networks. Deliver community outreach and presentations at senior centers, assisted/independent living communities, adult day care centers, and professional events to increase awareness of the Grey Matters and GUIDE models. Collaborate with the Solcere team to improve patient outcomes and support caregivers. Use CMS-approved contracts to onboard community partners. Qualifications: Licensed Nurse Practitioner (NP) in California. Experience in geriatrics, cognitive health, neurology, or related fields preferred. Strong interpersonal and communication skills with a passion for patient advocacy and community engagement/marketing. Ability to take initiative, build partnerships, and manage program development. Comfort with public speaking, relationship building, and acting as the face of the GUIDE program in the community. Preferred Skills: Background in marketing, outreach, or program development within healthcare or community-based settings. Experience with event coordination (community talks, professional presentations, workshops). Knowledge of CMS reimbursement structures, contracts, and reporting requirements. Bilingual (Spanish/English) to serve a broader patient population. Familiarity with electronic health records, data tracking, and program evaluation. Ability to work both independently and collaboratively in a dynamic, mission-driven environment. Why Join Us?
