We are currently seeking a Nurse Care Coordinator to join our dynamic team. Salary: $90, 000 to $100,000 Pay Period: Sunday to Saturday Pay Schedule: Weekly, Every Thursday via Direct Deposit Responsibilities Coordinate and manage patient care across multiple providers and settings to ensure seamless service delivery. Guide patients through the healthcare system, providing resources and support to address their needs. Assess patients' medical history, needs, and preferences to develop personalized care plans. Facilitate communication between patients, family members, and healthcare team members to ensure clear understanding of care strategies. Monitor patient progress, adjusting care plans as necessary to meet changing health conditions. Educate patients and their families on treatment plans, health management, and available community resources. Participate in quality improvement initiatives to enhance care coordination processes and patient outcomes. #IND1 Requirements Current and valid state licensure as a Registered Nurse (RN). Bachelor of Science in Nursing (BSN) preferred. At least 2 years of clinical nursing experience, with prior experience in care coordination or case management strongly preferred. Excellent assessment, problem-solving, and critical thinking skills. Strong interpersonal and communication abilities to build rapport with patients and healthcare team members. Knowledge of healthcare regulations, policies, and community resources. Ability to work collaboratively within a multidisciplinary team to enhance patient care.
