Call Tiffany at (208)695-6644 ______________________________________ Breakdown of Funds: Medication start fee $500 each injectable medication start (up to 3 times)= $1,500.00 Transfer fee $1,00.00 for each transfer (up to 3 transfers)= $3,000.00 Travel for transfer (if needed) hotel, food per dium of $75.00 a day for surrogate and $75.00 for surrogates companion, and airline tickets for surrogate and companion, transportation approximately $3,500.00 for up to 3 separate transfers if the 1st transfer doesn’t work= $10,500.00 Mileage reimbursement after the 50 mile of $.63 (or current IRA mileage rate) Health insurance monthly premiums estimate $500 a month for 1 year= $6,000.00 Maximum out of pocket medical expenses on health insurance policy for the year average policy is $7,500.00 for individual. Fertility medications to prepare your body for transfer estimated cost $3,000. Per transfer for up to 3 separate transfers if the 1st transfer doesn’t work= $3,000.00+ EACH transfer Lawyer legal fees to review your “Gestational Carrier” contract with you $1,500.00 for initial review then towards birth time $500.00 to file court documents so parents can take the baby home from the hospital $500.00 = $2,000.00 Maternity clothing allowance $700.00 Monthly non-accountable funds You’ll receive non-accountable $300.00 a month once legal contacts are completed to cover any local mileage (under 50 miles), vitamins etc. until you give birth. 12 months x $300.00= $3,600.00 Housekeeping in the last 2 months of pregnancy if chosen to use. Maximum of $400 a month for 10 months= $4,000.00 Individual counseling if you feel you need it up to $400.00 a month = $4,000 Escrow, all of your funds set up in a separate escrow ahead of time so you will be rest assured you will receive monthly ALL funds for your entire journey= $1,800.00 Surrogates base fee $40,000 - $48,000 (surrogates decide what they want for their fee agency DOES NOT decide) for first time surrogate and $50,000.00 - $70,000+ for repeat surrogates (once again surrogate decides their fee there is no set dollar amount). Payments broken down into 10 payment after heartbeat is established. For example if your base fee is $45,000 you would received $4,500.00 every month you are pregnant after heartbeat. Any remaining funds are paid in one lump sum after birth Total $92,600.00 Compensation for 1st time surrogates $45,000.00 (you select your fee but hypothetically say $45,000.00)
