The Multicultural Community-Based Patient Navigator/African American Services Coordinator directly assists clients with cancer in different settings, including public hospitals/health centers, community clinics, other community-based agencies, and in the home. In addition, this position conducts new client intakes and makes referrals to appropriate WCRC programs and services. This position will work primarily with African American clients, working in close collaboration with the Community Engagement Director/Support Group Facilitator, and other program/administrative staff. This is a part-time, non-exempt position, with some evening and weekends required, and this position reports to the Program Director. Navigation: • Conduct client intakes and assessments for navigation services; make intra and inter-organizational program referrals as appropriate; accompaniment to appointments, provide transportation, and assist clients in developing questions for medical teams and other care providers • Assist clients in understanding their diagnosis, test results, and treatment options • Facilitate multiple care aspects (case coordination, information sharing, one-on-one support, follow-up) • Help clients make informed decisions by acting as their advocate regarding treatment options and medical compliance • Provide direct assessment, personal emotional support and referral to cancer education, financial assistance, survivorship, and other programs • Maintain up-to-date understanding of cancer care (symptoms, treatment, wellness supporting techniques) • Meet with community stakeholders to communicate the benefits of WCRC navigation services on the lives of their clients • Provide patients with information to community resources to assist with needs • Conduct outreach to establish and maintain positive working relationships with medical personnel and community agencies • Facilitate client utilization of Season of Sharing program • Provide support and services such that they are in line with WCRC’s commitment to cultural humility and the organization’s core values (community, compassion, and change) • Other duties as assigned by the Program Director Community Building: • Collaborate with staff team in developing community presentations promoting WCRC services for African American community members and audiences. • In coordination with the Community Engagement Director/Support Group Facilitator, identify health needs of the African American population through client support and networks to assist in developing wellness classes for the African American community. Evaluation and Data Management: • Maintain accurate program data for ongoing program analysis/reference, evaluation, and development • Timely submittal of program data including intake, group attendance, and navigation hours Qualifications and Experience: • BA/BS and/or 3-5 years patient navigation, case management, or equivalent experience in a community setting • Proficient in Microsoft Office Suite, comfortable with client-specific data entry • Experience dealing with chronic illness, grief, death and dying • Skilled at working with individuals that have life challenges that come with being under-resourced • Understanding of cancer and/or women’s health issues • Must be self-motivated, energetic, and committed • Must be comfortable communicating by phone, email, and text • Ability to work independently as well as in a team setting • Ability to work flexible hours • Demonstrates sensitivity and comfort working in a culturally diverse environment • Ability to communicate effectively with staff, volunteers, clients and outside agencies • Must have excellent verbal and written communication skills Visit our website for additional information: https://www.wcrc.org/about-us/careers/

PEOPLE WITH DISABILITIES FOUNDATION (PWDF) is seeking a Program Director (PD) for Public Awareness and Education. PWDF is a small nonprofit agency, with a focus on equal access for individuals with psychiatric, developmental, and/or neurological disabilities. The PD is responsible for managing operations relating to public awareness and education activities that destigmatize psychiatric and/or developmental disabilities and advocate for the civil rights of people with these disabilities. Responsibilities include arranging live and live stream seminars, workshops and trainings; producing training materials; promoting events; writing, editing, and publishing electronic newsletters; arranging guest speaking; and managing the expansion of agency communications across all media, including website, live stream, traditional media, social media, and others to destigmatize psychiatric and developmental disabilities. Responsibilities also include networking; outreach; updating website and related administration; organization and maintenance of databases; hiring and supervising program staff and managing HR issues; administering vendor contracts, e.g., marketing PWDF’s educational videos; oversight of program income and expenses; preparing reports for management and the Board of Directors and related budget development; administrative support for program operations; and program compliance to internal standards, including filing and other organizational tasks to ensure successful program operations. Other duties as assigned. Successful candidate needs to be proactive, analyze data, processes, and information; and able to multitask. The PD works under the supervision of the Executive Director and may supervise at least 1 AA. Minimum Qualifications: Bachelor’s degree and minimum 3 years’ program management experience in a related field such as in education, communication, marketing, mental health, program development, nonprofit management, and/or social services, or equivalent. Experience related to nonprofit civil rights, disability rights (especially mental health) helpful. – Superior organizational, written and oral communication skills – Ability to work under deadlines – Ability to manage competing priorities – Ability to work independently and to exercise appropriate level of discretion and judgment – Experience and skill collaborating in a team setting with staff at all levels – Microsoft Word, Excel, and WordPress required.
