Responsibilities and Initiatives To help NHF meet its growth goals, the Social Services Coordinator will: Timely complete Needs Assessment and Care Plans. Complete referrals and coordinate services for mental health, housing, etc. Facilitate discharge process and locating housing for individuals. Complete discharge notes, communicating to our community partners. Conduct warm hand off for guest when appropriate. Maintain up-to-date CRM proprietary database daily and partner contacts. Complete Coordinated Entry System ("CES") Documents and all other documents required to maintain compliance. Complete Homeless Management Information Systems (HMIS). Utilize bed board application to determine census capital for operational planning (i.e.) meal planning. Provide emergency service referrals (e.g., domestic violence, human trafficking & sexual assault) and warm handoff referrals when appropriate. Attend care coordination meetings. Collaborate with nursing team to execute weekly case conferencing for guest. Practice harm reduction practices while conducting guests searches for contrabands or illegal substances when applicable. Support program team with appropriate guest interventions such as de-escalation, Crisis Prevention, motivational interviewing, etc. Engage and conduct daily check in with guest, building rapport and trust with our guests. Ability to adequately manage caseload of a minimum 25 guests with flexibility to manage more to meet the needs of the facility (i.e., vacancies, PTO etc.). Flexibility to complete home visits when necessary to track post-transition guest progress and or Rapid Rehousing Housing (RRH) when applicable. Must adhere to administrative hours to complete all required documentation. Project a positive professional image and adheres to organizational dress code. Ability to multi-task and collaborate with external case managers to provide adequate services and support to the guests. Ability to triage and intervene during areas of guest behavioral concerns. Adhere to all funders and company’s standards procedures of operations, regulations, and best practices to meet compliance. Flexibility to adapt to schedule changes when necessary to meet the needs of the facility. Complete additional tasks/duties as assigned to meet the needs of the facility, notwithstanding primary responsibilities. Always follow NHF policies and procedures. Requirements Bachelor’s Degree Required; Master’s in social work preferred. Minimum of three (3) years relevant experience in healthcare delivery, hospital education, social services, or similar field. Positive and professional attitude. Time management and strong organizational skills. Computer proficiency in MS Word, Excel, Outlook, PowerPoint, and proficient typing skills. Negotiate terms of Lease Agreement v. Assist Eligible Consumers to make informed decisions. b. Housing Assistance The RS shall ensure the following: i. Execution of the Eligible Consumer’s Subsidy Assistance Payments Contract through SPC or other State or Federal Housing Agencies. ii. Maintain current information of Eligible Consumer benefits, i.e., basic social security information, medical etc., to ensure housing assistance is current and accurate in regards to the Eligible Consumer’s income. 3. Provide housing advocacy and other supportive services to Ohana CCS/AMHD’s targeted population that are authorized through Ohana CCS’s Behavioral Health Authorization Division or the AMHD’s Utilization Management Division a. Assist Eligible Consumers to identify and implement individualized housing related goals, objectives and performance measures. i. Initial and ongoing evaluation of Residential Service Plan. ii. Responsible for assisting the Eligible Consumer in achieving performance outcomes of SHP goals and objectives. b. Maintain collaboration with designated Ohana CCS/AMHD approved Service Providers to assure Eligible Consumer’s stability in the home. i. Initial and ongoing evaluation of Housing Assessment. c. Assist Eligible Consumers in meeting tenancy requirements i.e. payments and other lease requirements etc. d. Assist Eligible Consumers with housing related issues relevant to fulfilling lease requirements. i. Initial household set-up ii. Budgeting iii. Housekeeping iv. House rules v. Contacting the landlord regarding repairs or other problems in the home. vi. Health and safety (including fire emergency plan and other safety and security concerns) and other issues e. Help mediate landlord/tenant issues. f. Provide follow-up services to Eligible Consumers in maintaining their homes by providing periodic in-home visits. g. Provides regular encouragement to Eligible Consumers. h. Advocates and supports Eligible Consumers with enthusiasm and professionalism. i. Assists Eligible Consumers to apply and/or transition to other types of “long-term” housing assistance, such as mainstream Section 8, Rent Supplement, or Public Housing. j. Assistance, guidance, and referral to other resources. B. Administrative Responsibilities 1. Maintain security and confidentiality of Eligible Consumer information, which includes but is not limited to I.D. date, social security numbers and financial information. 2. Prepare internal reports for SPC which include but are not limited to: demographic information, services provided to Eligible Consumers, appointments with landlords, property managers, housing subsidy programs, marketing strategies, meetings with Eligible Consumers, Ohana CCS, AMHD and other service providers. 3. Administers standardized Eligible Consumer self-report measures i.e. Lehman, Quality of Life Interview, Brief Version and the MHSIP Consumer Satisfaction Survey. 4. Coordinate and record Eligible Consumer applications to subsidy programs such as Section 8, or other types of permanent housing. 5. Maintain accurate daily log of activities submitted for review by SPC. 6. Participates in SHP team meetings, SPC committees as assigned and attends Service Provider staff and/or treatment team meetings as necessary.
