This will be a very rewarding experience for reliable people who have a passion to help others. A perfect part-time or second job! No experience necessary. We are willing to train the right candidate. Flexible availability. 25 - 40 hours a week (may include weekends) Rate: $22.25-23.25 SUMMARY Provides instruction, coaching, assistance, support, and advocacy needed by individuals with developmental disabilities (members) to successfully work in a community job setting. ESSENTIAL DUTIES AND RESPONSIBILITIES include, but not limited to the following: Orients the member to the work setting. Develops a daily work schedule that identifies all of the job duties that must be completed during the work day. Trains the member to complete their assigned job duties up to employer expectations. Teaches the member appropriate workplace behavior and attitudes. Assists the member develop relationships with co-workers. Requirements Must speak English High School Diploma or equivalent Experience with special needs / developmental disabilities (preferred) Driver's License for 3+ years required. Must be at least 21 years of age. Must be willing to drive an 8-passanger van. Authorized to work in the United States. Conduct community-based outreach and education on healthcare services, disease prevention, and healthy lifestyles. Advocate for member and community needs within healthcare systems and social services. Establish partnerships with community-based organizations to broaden the range of support services available to our members. Provide culturally appropriate health education on topics such as nutrition, chronic disease management, mental health, and preventative care. Support individuals in understanding and navigating the healthcare system. Organize workshops and informational sessions on healthcare topics. Assist individuals in accessing medical care, insurance programs, and social services. Help members close care gaps by scheduling appointments and reminders, assisting with paperwork, and understanding their treatment plans. Identify barriers to healthcare (SDoH) and provide resources. Connect individuals with social services, such as housing, food assistance, and employment support. Document interactions in Wider Circle’s digital systems in a timely manner. Comfortably stand and speak publicly, projecting your voice in indoor and outdoor spaces. Sit, stand, and walk routinely during events to interact effectively with members. Ability to sit for extended periods while conducting member outreach by phone. See and hear well enough to interact with members in-person or by phone and respond to member questions. Use a computer to view, enter, and update information in different systems. Effectively communicate with members in-person or virtually. Requirements A strong passion for helping and serving others. Outgoing personality and unwavering, positive attitude, with the ability to engage with others in-person and via phone or video conferencing. A genuine interest in working with Medicare/DSNP/Medicaid populations and a commitment to building strong, supportive relationships. Strong verbal and written communication skills to effectively engage with diverse community members. Proactive approach to outreach, with the ability to take lead on community engagement initiatives. Patient Duties Responsibilities: • Evaluate participant’s health status and needs by obtaining a health history and history of problems or treatments related to the feet. • Perform assessment to determine if there are abnormalities of the feet and whether other health care is needed such as referrals to other health care professionals. • Treat feet, including inspection and palpation, alleviation of dry skin problems, and trimming toenails. You will set your own work schedule, work from home, and you will have a lot of flexibility. We are seeking someone for 10 to 15 hours per week. This is a short term project, you will be working from about now until the 3rd week of December. The pay is not high, but we are looking for someone with high skills. The right person will take this position as much for the money as for the impact. The impact is we are starting up a "home food donation" system across 6 counties. Our non profit provides an operating system for households to leave a bag of food on their doorstep, and for volunteers to pick that food up and take it to pantries. Food Insecurity is a huge problem for our area, and this program could result in more than 1 million pounds of food going to food banks and pantries. Some example of things you'll be doing; 1) proficient in our operating system, teaching others how to use it, 2) planning and organizing, perhaps even designing, marketing materials. 3) organizing table top events at grocery stores, fairs, and other community events, and lots of other miscellaneous work.
