





About the Role: We're seeking a dedicated Medical MAP Case Manager to lead and ensure compliance with the Medication Administration Program (MAP) across several residential programs, including ITR, CTR, Phoenix Rising, and Pre-Independent Living. This role is essential to ensuring participants’ medical needs are met and MAP policies are followed consistently.What You’ll Do: Lead weekly MAP audits and medication assessments across all programs Collaborate with Program Directors, staff, and providers to ensure proper medical care Ensure daily, weekly, and monthly MAP documentation is completed accurately Support staff with state MAP requirements and medication training Monitor med rooms, medication counts, and ensure proper documentation (e.g., MARs, narcotics logs) Handle medication orders, disposals, and transcription oversight Attend DCF audits and assist in audit preparation Participate in MAP on-call rotation Medical Oversight Includes: Manage resident medical appointments and paperwork Maintain up-to-date medication lists and HCSPs Support clinicians with referrals, consents, and required documentation Communicate with providers regarding medication changes or issuesTrack and prepare for next-day appointments (encounter forms, insurance, consents, etc.) Competencies & Qualifications: Valid Driver’s License & Acceptable Driving Record MAP Certification (required) and MAP Site Supervisor Certification (can be completed after hire) Background or education in healthcare preferred Experience working with youth and families, with a strong understanding of strength-based care and crisis intervention Strong leadership, organization, and the ability to prioritize and adapt in a dynamic environment Excellent professional communication skills Training & Compliance: You'll complete all required compliance trainings (CPR/First Aid, Trauma-Informed Care, HIPAA, Ethics, etc.) and stay up to date with refresher courses offered by Centerboard to maintain certifications and ensure top-quality care.
