Malama Pono Health Services is searching a Case Manager to provide health insurance navigation services to the uninsured Kauai population.. Duties and Responsibilities: *Complete all training and turn in all paperwork required by Department of Human Services (DHS), Med-QUEST Division (MQD), *Centers for Medicare & Medicaid Services (CMS) and Federally Facilitated Marketplace (FFM). *Attend meetings as required by Malama Pono Health Services (MPHS) and MQD. *Assure timely compliance with all contract responsibilities. *Travel to consumers in communities and provide the primary role of assisting people by providing information on affordable health insurance and direct assistance through the enrollment process *Build relationships with other community organizations *Provide feedback to Program Manager on strategies for effective engagement with community members and the type of appropriate tools for educating specific target populations *Track and record data of numbers of consumers reached through presentations, community events and health fairs, the types and numbers of other consumer assistance provided and number of people enrolled Requirements: *Basic computer skills, including Microsoft Office suite (Word, Excel, Outlook, etc.). *Good interpersonal, written, and verbal communication skills. *Ability to prioritize workload and daily activities and complete tasks in an efficient manner. *Ability to establish and maintain effective working relationships with staff, clients, and outside contacts from a wide variety of ethnic, socio-economic, and cultural backgrounds. *Dependable, able to work under pressure, receptive to change, and willing to learn. *Non-judgmental attitude and ability to respect the knowledge, actions, and feelings of others. *Demonstrated ability to work productively, both independently and as part of a team. *Ability to work well and thrive professionally in an atmosphere of significant diversity, working with marginalized populations. *Responsibility for maintaining client confidentiality and PHI *Effectively trained in cultural competencies Preferred Qualifications: *Extensive knowledge of Kauai. Required Work Cards/Certifications: *Valid Hawaii Driver’s License and clean three-year (3-year) Driver’s Abstract. *TB Clearance
