Schedule: • Shift: 9:00 AM - 5:00 PM • Travel: 70% office-based; travel to patient homes (Manhattan, Brooklyn, Bronx, and Queens) for wound care consults as needed • Caseload: 15-20 consults per day Responsibilities: 1. Assess and treat complex wounds, ostomies, pressure injuries, burns, and diabetic foot wounds. 2. Collaborate with an interdisciplinary team to develop individualized, evidence-based care plans. 3. Provide patient and family education on wound care, infection prevention, and treatment measures. 4. Participate in the development and revision of wound and ostomy care policies and protocols. 5. Train and evaluate field nurses on wound care techniques and documentation. 6. Conduct quality improvement initiatives, case load reviews, and chart audits. 7. Ensure compliance with state, federal, and agency policies and procedures. Requirements Qualifications: • License/Certifications: • Active New York State Registered Nurse License • Wound Care Nurse Certification or Wound Ostomy Care Nurse Certification • BCLS Certification • Education: Bachelor’s Degree in Nursing (required) • Experience: • Minimum 2 years in a Certified Home Health Agency or post-acute care setting • 2 years of medical/surgical experience • 2 years of public/community health experience • Familiarity with performance improvement initiatives and relevant software applications Skills and Technical Requirements: • Knowledge of EPIC H2O, Microsoft Office Suite, and PI applications • Experience with wound care equipment, including Varisource/HDR and CT Simulator • Strong verbal, written, organizational, and analytical skills Requirements: • Flu vaccine required (medical/religious exemptions accepted) • COVID-19 vaccine not required Benefits Compensation: • Pay Rate: $91/hour

Home Health Aides work in patient homes and in facilities. Home Health Aides possess the ability and willingness to interact with residents, family members, personnel, etc., under all conditions and circumstances. Schedule: Monday-Friday, 7a-3p or 8a-4p Area: Philadelphia, PA Job Responsibilities Providing frail, ill, or immobile clients with assistance, company, and comfort. Executes the patient care plan and communicates well with RN Case Manager. Monitoring the physical, mental, and emotional conditions of clients and reporting changes to designated parties. Performing personal hygiene care Performing light housekeeping duties in patient's living area only. Is aware of and follows nutritional guidelines for patients. Preparing meals and assisting clients with eating and drinking. Assisting with the use of incontinence products or providing bathroom visit support.
