Keeping Families Together is a long-term program aimed to assist DCPP involved families challenged with homelessness/housing instability putting children at risk for out-of-home placement or delaying reunification. KFT utilizes a housing-first model to work with our families in obtaining stable, subsidized housing. The program focuses on the following to help families overcome the obstacles that prevent the family unity they desire: 1) Reunification; 2) Housing Stability; 3) Vocational Counseling; 4) Substance Use Services; 5) Socialization and Self-Sufficiency; 6) Parenting Skills; 7) Strengthening Families Curriculum. We are currently hiring Social Workers - Early Start Assessment to lead the assessment of applicants (0-36 months) who are applying for services under the Early Start program. Facilitate IDCT eligibility assessment coordination and for those found eligible develop Initial IFSP within 45 days of referral. This role offers a hybrid schedule giving you the opportunity to work in with clients in their homes in San Mateo County, from our San Mateo office and from the comfort of your own home. Local travel within the county lines of San Mateo will be required. Access to reliable transportation will be needed. Responsibilities Initial Early Start Applicant Assessment Contact the client/family/guardian and makes an appointment to meet with them. Interview for purposes of obtaining social, emotional, developmental and familial information to form a home based assessment. Obtain the completed intake information from client and/or family and conducts a need assessment prioritizing available resources selections. Clarify the request for services and the needs of client/family. Review and analyze available data; identify barriers and opportunities in clients’ developmental objectives and plans. Design, monitor and revise Individual Family Service Plan (IFSP) to meet personal goals and determines client availability within the state mandated timeframes. Work with client families to assess needs, preferences and life choices. Confirm any immediate needs and provide or arrange for any necessary interventions or services. Meets with the applicant/family/guardian within a designated time period and reviews submitted materials. Obtain the completed application form from applicant and/or family (or assist in completion of form). Provide information regarding Golden Gate Regional Center, Early Start and services available. Interview for purposes of obtaining social, emotional, developmental and familial information to form social diagnosis Completes Early Start Assessment report Schedule and coordinate meetings with team and applicants Obtain necessary information to complete Applicant Developmental Evaluation Report (CDER) Obtain any additional signed Release of Information forms for additional data base material Submit completed application packet and completed Releases of Information to the Office Assistant – Intake Coordinate assessment team's completion of the CDER at the time of eligibility determination Provides information and referrals for applicants who are not eligible for agency services Research available community resources and services Ensure that services are provided per the Lanterman Act and Title XVII Obtain or collect information or data for use by interdisciplinary team. Access Services is hiring an empathetic and motivated Family Support Specialist for the Bucks County LIFE program who will serve families across Bucks County, and be based out of our office in Warminster, PA.   As a Family Support Specialist, you are a parent or caregiver of a child who has received services for behavioral health challenges, intellectual or developmental disabilities within Bucks County. Because of your lived experience, you will be a trusted support to families helping to maintain their own wellbeing as they support their children, find and navigate the resources and services available to their children in Bucks County. You will empower families to successfully partner with the agencies and school systems supporting their child in a team environment. In addition, you will help facilitate support groups and training.   The Family Support Specialist will participate in county and program initiatives, outreach, and meetings. You may partner with the Director of Bucks LIFE, Administrative Case Manager or Certified Peer Specialist in order to provide direct family training and support and create a team environment.  Work Schedule: Full Time, 40 hours per week, M-F with occasional evenings and weekends for urgent support needs and/or events.  Flexible schedule established to meet the needs of families receiving support.  This job requires travel across Bucks County to meet with families in person to support their needs, with mileage reimbursement for travel between appointments while on the job. Requirements Key Responsibilities of the Family Support Specialist: Help navigate all barriers, including cultural, access and understanding of Bucks County systems of care for children with additional needs.   Provide essential parent support and mentoring, acting as professional staff liaison between parents and the child serving systems.  Share parenting experiences/skills through individual support and training about what works and what does not work in parenting special needs children and navigating the systems of care.  Support children and their families through phone contact, digital technology, and direct service  Participate and accompany families to plan care meetings, team meetings, school meetings and other appointments as needed.  Facilitate groups and parent training, and special projects for family voice and community outreach.  Assist families to navigate systems that meet their needs and/or their children's needs, and maintain excellent documentation using an electronic health record. Support families with unique needs such as co-parenting through divorce, transition age family dynamics, grandparents as parents, mental health and wellbeing supports.   Work with county offices of Children and Youth, Juvenile Justice, Mental Health and Intellectual/Developmental Disabilities and other collaborative agencies to offer support and continuity of care.  Travel during the course of a normal workday to attend offsite meetings, provide services in the community. Therefore, the employee will need to drive their personal vehicle to meet these expectations, drive in a safe manner at all times, and abide by the Motor Vehicle Driving policy at all times.   Requirements and Qualifications of the Family Support Specialist: Education: High school diploma or GED required. Certification: Parent Support certification preferred. Experience: Experience as parent/family member/caregiver of a child with behavioral health challenges, intellectual or developmental disabilities, and involvement in Bucks County services required. Experience working with diverse populations and providing culturally sensitive services is required.
