




Overview: We are seeking a dedicated and proactive Case Manager to support young parents in our program. This role provides direct case management, coordinates services, maintains accurate records, and ensures clients achieve their treatment and independent living goals. The Case Manager works closely with the Program Director, clinical staff, and external partners to deliver high-quality, client-centered care. The schedule for this role will be Monday through Friday, 9AM to 5PM (or a similar shift) on weekdays.Key Responsibilities: Provide direct support to young parents and families, including intake, assessments, and ongoing case management. Coordinate client intake, transportation, and orientation, ensuring timely completion of all required documentation. Develop, implement, and monitor individualized treatment and safety plans, including updates every 30–90 days. Maintain accurate client records and documentation in compliance with program, state, and regulatory standards (EEC, DCF, etc.). Collaborate with multidisciplinary teams, including clinicians, DCF, school staff, and community partners. Monitor and support clients’ daily living skills, budgeting, education, employment, housing, and health needs. Lead and facilitate groups and workshops on parenting, safety, and independent living skills. Serve as a role model and provide mentorship to both clients and staff. Participate in on-call rotation, respond to emergencies, and document incidents in program systems. Support staff training, supervision, and quality improvement initiatives. Assist with discharge, transition planning, and referrals to community resources. Qualifications: Experience working with youth and families in social services or related fields. Strong organizational, documentation, and communication skills. Ability to work independently, manage multiple priorities, and collaborate with diverse teams. Valid driver’s license and reliable transportation. Knowledge of DCF, EEC, and youth service regulations preferred. Bachelor’s degree in psychology or a related field, with at least 2 years of experience in residential or youth services. Flexible schedule to meet client needs, with strong communication, organization, and problem-solving skills. Ability to manage behaviors using collaborative and proactive approaches. Required Skills & Training: Willingness to be trained in: CPR/First Aid, Crisis Dynamics & Physical Restraint, Medication Administration (MAP), HIPAA, OSHA, Ethics & Boundaries, Trauma-Informed Care, Collaborative Problem Solving, and DEI engagement. Strong prioritization, adaptability, and professional communication skills.
