JOB SUMMARY The Minimum Data Set (MDS) Coordinator is responsible for the MDS assessment process completion, care planning process, and scheduling of MDS assessments. The MDS Coordinator assures maximum reimbursement by: determining payment coverage of each resident from admission to discharge; and by strategically setting Assessment Reference Dates (ARD’s) for the MDS in collaboration with therapy services within the Omnibus Budget Reconciliation Act (OBRA) requirements. This position is responsible for determining Resource Utilization Group (RUG) levels and assisting with admission assessment processes. This position requires on-going education related to rules and regulations regarding MDS process and reimbursement. JOB RESPONSIBILITIES Administrative functions May be involved in the pre-admission assessment in collaboration with the DON, Admissions Coordinator and HIM to determine coverage level for Skilled Nursing Facility (SNF) stay. Meets routinely and as indicated with the business office regarding billing requirements. Leads PPS meeting with therapy and unit coordinators to ensure goals are accomplished, review resident's progress, determine reimbursement days used, days left to be used, and overall plan of care. Coordinates with interdisciplinary team (IDT) dates of discontinuation of qualified Medicare coverage benefits for stay and date of termination notification issued to resident and/or responsible party. Tracks to ensure necessary certifications/re-certifications are completed and obtained in a timely manner. Monitors regulatory compliance for completion of MDS/Resident Assessment Instrument (RAI) process. Attends morning IDT meeting, care plan, and discharge meetings as indicated. Resident Assessment Instrument (RAI) process MDS submission/transmission Quality assurance Other functions Supports Nursing staff in their absence. Attends ongoing educational programs relative to Medicare, Prospective Payment System (PPS), MDS/RAI process, reimbursement changes, and Centers for Medicare and Medicaid Services (CMS) regulatory revisions. Provides educational training relating to Medicare, PPS, and MDS/RAI process to staff as requested. Provide direct nursing care as necessary. Requirements SKILLS, ABILITIES AND EXPERIENCE Knowledgeable of the theory, methods, and techniques of nursing. Knowledge of Medicare and Medical reimbursement principles and methods of implementation. Knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions. Ability to maintain composure and cope with stress and frustration. Knowledge in computer skills with competency in computer applications. Ability to use office equipment and programs including but not limited to: email, telephone, computer, photocopier, printer, fax and Microsoft Office programs Ability to communicate clearly in oral and written English. Documents approved overtime on employee time adjustment forms. Directly supervises nursing staff on assigned shift. Provides staff feedback on performance. Contributes to formal introductory and annual employee evaluations as prepared by the CNO or Nurse Managers.  Supports the CNO, Director of Nursing, Nurse Manager, and nursing staff to provide a high-quality orientation and ongoing education for clinical staff. Continually educates staff on key issues relating to hospital policies and procedures and health care regulations. Ensures accurate and timely shift-to-shift reports. Monitors clinical issues for high acuity patients or patients with unusual problems. Coordinates off site tests, emergency room visits, admission and discharges, lab processes, work orders and audits. Addresses and resolves patient and family complaints. Proactively works with NM and other hospital leaders when a complaint becomes a potential grievance and alert the hospital’s Internal Patient Advocate Requirements Education/Licensure Current license as a Registered Nurse in the State of California. Current Cardiopulmonary Resuscitation (CPR) certification required, or obtained within initial orientation period.  Verbal de-escalation and physical intervention program as approved by the CNO. Knowledge and experience of psychiatric diagnosis and associated risks, medications, biological systems, nursing process, treatment planning, and leadership. SKILLS AND COMPETENCIES The House Supervisor will pass initial and annual competency evaluations including (not limited to): suicide prevention, safe use of restraints, de-escalation interventions, medication knowledge, code blue/emergency interventions, infection prevention, and appropriate use of seclusion and restraints. The House Supervisor will need to complete additional competencies related to managing patient care, staffing systems, and internal reporting requirements. The House Supervisor has a high level of knowledge of Policies, Procedures, Unit Operations, Hospital Operations, Health Care Regulations and educates staff on key issues. The House Supervisor will demonstrate that, in addition to being competent, they are skilled in applying knowledge and competency in their daily nursing practice.   PHYSICAL REQUIREMENTS While performing the duties of this job, this position is frequently required to do the following:  Stoop, kneel, crouch, reach, and stand for sustained period of time. Walk, push, pull, lift, carry objects from a lower to higher position or horizontally from position to position or otherwise move objects. Feel sizes, shapes, temperatures, and textures by touching with skin, particularly that of the fingertips. Express or exchange ideas orally and potentially loudly, accurately, or quickly. Perceive the nature of sound with no less than a 40 db loss @ 1000 Hz and 2000 Hz with or without correction. Able to provide manual and mechanical restraints for patients who are physically acting out and are potentially dangerous (at times this occurs on the floor) Minimum standard of visual acuity with or without correction. While worker may possibly be subjected to temperature changes, the worker is generally not substantially exposed to adverse environmental conditions as the work is predominantly inside.
