Medication Technician - Experienced
General Description: The primary obligation of the Medication Technician is to provide excellent personalized care to all residents in the context of medication management. Communication with residents, families, and health care professionals will build and maintain a strong health care network that focuses on optimizing each resident’s health and care plan. Certification Requirements The Medication Technician must complete the initial training program including hands on and Relias training. Must stay current on all Relias annual training. ESSENTIAL FUNCTIONS: A) Medication Management/Documentation 1. Manage medication dispense directly to residents as prescribed. 2. Supervise residents to ensure medications are taken. 3. Document medications administered to residents on resident’s MARS, including medication refusal, medication holds, etc 4. Reorder medications in a timely manner, i.e. PRN medication, narcotic medications that are not part of the monthly cycle. 5. Record newly delivered medication on centrally stored medication logs. Maintains paper copy in 622 folder, as well as the excel version in each residents file. 6. Maintain signed and updated Medication Verification Lists for each resident, i.e. residents file and emergency envelope. 7. Keep emergency envelopes updated. Ensure that extra copies are available in med room and residents file. 8. Maintain copies of all physician orders in 622 folders. 9. Create medication records, i.e. medication verification list, centrally stored medication record, PRN letters, and Mars for all incoming residents. 10. In absence of administrator on duty, assess potentially new residents, i.e. evaluating physical, cognitive, and emotional capabilities and care needs. 11. Monitor residents for change of condition and communicate changes and concerns to the appropriate health care provider. Requesting specific orders, medication changes etc. Documents all changes in resident’s medication records. B) Skin Management/ Documentation 1. Monitor resident’s skin condition, i.e. weekly checks in AM when residents are being showered watching for new rashes, bruises, skin tears etc. 2. Document all new skin conditions on resident’s weekly log and notify the appropriate health care professional to ensure immediate action. 3. Make daily rounds and checks residents who are known to have a skin condition. Ensure that patches are intact, wound barrier creams are used, etc. 4. Contact third party health care providers, such as In Home Care and Hospice if their services are needed, i.e. contact primary physician to get wound consultation referral. 5. Update and direct care staff in regard to resident’s skin care needs, i.e. prevention techniques such as continuous repositioning, use of heel protectors etc. 6. Keep Administrator and Assistant Administrator up to date on all important matters in regard to the residents’ skin condition care. DUTIES: 1. Maintain and monitor weekly care logs for all residents, ensuring regularity of BM, urine output, meal consumption, and skin issues etc. 2. Communicate any necessary changes in residents’ care to floor manager and other hands-on care staff, i.e. giving appropriate instructions in regards to changes of care procedures. 3. Check in with third party health professionals, i.e. physicians, nurses, PT, OT, and speech therapists during weekly visits to facility. Requests orders for appropriate equipment, i.e. walkers, wheelchairs, hospital beds etc. 4. Update residents POAs/ family members of important changes & new conditions. 5. Keeps Administrator and Assistant Administrator up to date on all important matters in regards to residents overall health condition. Physical requirements: 1.
