Access Services is hiring a Case Manager of Justice Related Services. You’ll work directly with justice-involved clients to support their reentry, stability, and growth, connecting them to vital resources and advocating for their success. This role is about more than casework—it’s about restoring dignity, building trust, and helping people reclaim their futures.  In this position, you will help clients achieve goals such as finding housing, gaining employment, developing a positive social life, using healthy coping skills, and improving their overall personal wellness. A successful candidate will be humble, open to coaching and teaching, flexible, able to work alone and with a team, open minded, able to critically think, and willing to advocate.  Essential Functions  Engage with adults experiencing severe mental illness using person-centered approaches, nonjudgmental support, and motivational interviewing to assess re-entry needs and support goal development and maintenance.  Coordinate care and deliver a warm hand-off to providers that the individual is being referred to ensure re-entry success and continuity of care while in the community.  Provide de-escalation as needed to support successful engagement and follow-up care.   Provide direct or indirect skill development. Provide information about mental illness, wellness, and recovery.  Assist persons being served and team members in increasing awareness of community resources.  Act as a point person for relational partnership and collaboration with all service providers involved.   Provide thorough documentation of work within Agency electronic health records.  Participate in the on-call schedule and provide support to participants as needed.  Engage participants in jail, in the community, at court, in hospitals and throughout all 5 intercepts.  Build collaboration through community outreach and awareness.  Attend county, committee and consumer meetings as needed.  Facilitate multisystem meetings and provide support for complex cases.  Attend regular team meetings and case reviews.   Write case review documents and facilitate discussion and review of services during staff meetings and/or case reviews.  Complete all outcomes, statistical reports and billing documents as needed.  Comply with licensing regulations under Blended Case Management and participate in the program audits.    Other:  Maintain regular and predictable attendance, and work scheduled overtime, or be available for unplanned overtime as necessary, to meet program needs.  This is a client-facing position that does not include remote or hybrid work schedule options. This job requires full-time, on-site presence due to the nature of the role and responsibilities. For more information, please refer to Access Services Policy and Protocol: Hybrid Remote Work Model Best Practices.   This position requires the ability to travel during the course of a normal workday to successfully carry out the expectations of this position including attending offsite meetings, providing services in the community, and providing service oversight and supervision if applicable. Therefore, employee will need to drive their personal vehicle and/or an agency owned vehicle to meet these expectations. Employee is expected to drive in a safe manner at all times, meet eligibility criteria outlined in our Motor Vehicle Policy, be available and willing to drive during work schedule, and successfully pass the agency’s substance abuse screening at any time during employment. There may be limited exceptions, which should be identified and worked through with supervisor.    Requirements Education: Bachelor’s degree in social work or a related field required.  Experience: Experience working with adults with serious mental illness and the criminal justice system preferred.  Knowledge, Skills, and Abilities:   Ability to communicate well.   Ability to develop positive, creative and programmatic goals.   Ability to direct and prioritize own work and be flexible.   Knowledge of criminal justice system. Knowledge of serious mental illness.   Ability to work as part of a team.   Proficiency in, or ability to learn and use, workplace applications, platforms and technologies.  Ability to navigate network drives and basic hardware use and troubleshooting.  Ability to use electronic health care record system.     Essential Working Conditions/Physical Demands: Work in program sites and the community routinely providing direct care and/or support with individual(s). Stand, sit or run by self, or to provide proper care and supervision with an individual. Able to move and stay with an individual to keep them safe, both in home environment and community, both at near and far distances, even when an individual may struggle to meet identified expectations. Able to assist an individual who has fallen, regardless of weight or size. Walk, climb stairs by self, or to provide direct care and/or support. Work in a standard office environment. Utilize desktop computer equipment on a daily basis. Able to lift or move items associated with sedentary work and light work (exert up to 20 lbs.) Travel to offsite locations and regional office locations if necessary.  Benefits Pay: Starting rate of $20/hourly, with increases based on education and work related experience. The Senior Case Manager will carry a caseload at the hotel site. The position requires a client-centered approach that respects self-determination, involves them in decision-making, and empowers them to achieve their goals. Effective collaboration with community partners and service providers is essential for providing holistic support and addressing any gaps in care. The role also involves maintaining accurate documentation and preparing reports to ensure compliance with organizational policies and funding requirements. The role involves developing and implementing individualized plans to secure permanent housing.     The Senior Case Manager will lead the team in prioritizing housing solutions, collaborate closely with the assistant director of programs, and ensure that both on-site and hotel locations maintain the necessary capacity to meet community needs. Additionally, the Senior Case Manager will provide direct support to Case Managers to enhance service delivery.    Responsibilities:   Participant-Centered Support and Supervision:  Carry a caseload of participants at the off-site hotel location  Develop individualized housing-first service plans and guide participants through ending their homelessness:  Include housing location service, referral, and advocacy  Facilitate housing-relation tasks such as collecting necessary documents  Assist with developing a rental assistance package for participants utilizing a variety of funding sources, properly document the distribution of funds, and lead the recertification process if additional funds to maintain housing   Develop and implement individualized plans to help participants secure and maintain permanent housing.  Assist participants in securing basic needs items and services, including clothing, employment, childcare, etc.  Empower and support participants in the successful completion of their identified goals.  Provide services that are convenient and accessible for participants, including off-site visits.   Lead the case management team in providing effective services to the participants  Provide direct support to Case Managers, including phone screenings, diversion support, and guidance on complex cases.  De-escalate any safety concerns promptly and compassionately.  Complete and submit all documentation required for incidents before the end of the shift    Collaboration & Administrative Support  Function as a contributing member of a multidisciplinary team to ensure timely service delivery  Assure that all relevant information about participants is disseminated to all appropriate staff members promptly.  Monitors participants progress by regular, ongoing contact with individuals receiving services and contact with collaborative service providers both external and internal necessary assessments.
