Hospice Chaplain - Pastoral/Spiritual Counselor

This position involves working closely with a diverse group of patients in the AdvisaCare team located in Metro Detroit. Requirements Completed graduate theological studies from an accredited seminary or divinity school. Experience in hospice care or a related field is highly preferred. A demonstrated ability to provide comfort and support to grieving patients and families. Strong listening skills and a patient-centered approach. Willingness to perform home visits and collaborate with the interdisciplinary team. Critical thinking skills to adapt spiritual care practices to individual patient needs. Key Responsibilities: Conduct spiritual assessments to understand the unique needs of each patient and family. Develop personalized spiritual care plans in collaboration with local clergy if requested. Maintain compliance with all AdvisaCare policies, and applicable regulations. Provide spiritual care documentation in Electronic Medical Records accurately. Support team members in providing bereavement services as needed. Participate in team meetings and contribute to performance improvement initiatives. Uphold high standards of customer service and compassion in all interactions.
