Speech-Language Pathologists (Per diem)

Schedule: • Shift: 9:00 AM - 5:00 PM • Travel: 70% office-based; travel to patient homes (Manhattan, Brooklyn, Bronx, and Queens) for wound care consults as needed • Caseload: 15-20 consults per day Responsibilities: 1. Assess and treat complex wounds, ostomies, pressure injuries, burns, and diabetic foot wounds. 2. Collaborate with an interdisciplinary team to develop individualized, evidence-based care plans. 3. Provide patient and family education on wound care, infection prevention, and treatment measures. 4. Participate in the development and revision of wound and ostomy care policies and protocols. 5. Train and evaluate field nurses on wound care techniques and documentation. 6. Conduct quality improvement initiatives, case load reviews, and chart audits. 7. Ensure compliance with state, federal, and agency policies and procedures. Requirements Qualifications: • License/Certifications: • Active New York State Registered Nurse License • Wound Care Nurse Certification or Wound Ostomy Care Nurse Certification • BCLS Certification • Education: Bachelor’s Degree in Nursing (required) • Experience: • Minimum 2 years in a Certified Home Health Agency or post-acute care setting • 2 years of medical/surgical experience • 2 years of public/community health experience • Familiarity with performance improvement initiatives and relevant software applications Skills and Technical Requirements: • Knowledge of EPIC H2O, Microsoft Office Suite, and PI applications • Experience with wound care equipment, including Varisource/HDR and CT Simulator • Strong verbal, written, organizational, and analytical skills Requirements: • Flu vaccine required (medical/religious exemptions accepted) • COVID-19 vaccine not required Benefits Compensation: • Pay Rate: $91/hour

We are looking for a caring and gentle female Overnight Assistant / Night Aide (from 10:00 pm - 6:00 am) for our elderly mom who lives in a group home in Chandler (85286). As needed, the overnight assistant will help our mom go to the bathroom, help her get back into bed, massage her legs, and soothe her back to sleep. Pay is at the rate of $80 per night. Performing personal hygiene care Performing light housekeeping duties in patient's living area only. Is aware of and follows nutritional guidelines for patients. Preparing meals and assisting clients with eating and drinking. Assisting with the use of incontinence products or providing bathroom visit support. Running errands, including shopping for groceries and basic household items for patient only. Assisting clients from their beds into chairs or wheelchairs, and back. With prior approval by Staffing Coordinator, accompanying clients on trips, such as medical appointments. Maintaining a clean, tidy, and safe environment. Documents care using Agency approved Electronic Visit Verification system from the patient's care location. Consistently shows accountability to schedule and communicates all needs for scheduling changes to Staffing Coordinator.
