The Youth Transition Services Facilitator assists teens and young adults with disabilities, ages 14 to 22, to acquire knowledge, skills and supports necessary to prepare for a fulfilling, self-directed life after high school. Our mission is to enhance the independence and inclusion of people with disabilities through advocacy, information and referral, peer support, skills training, PCA, and transitional services.
