Trusted Path Home Health is a Joint Commission–accredited agency committed to providing compassionate, patient-centered care in the comfort of our clients’ homes. We are seeking an experienced Registered Nurse with a minimum of 1 year of home health care experience to join our dedicated team.

Responsibilities
The RN will deliver skilled nursing care and coordinate services for patients in their homes in accordance with physician orders, agency policies, and Medicare guidelines. Duties include:
• Conduct Start of Care (SOC), Recertification, and Discharge visits.
• Perform comprehensive patient assessments, including physical, psychosocial, and environmental evaluations.
• Develop and implement individualized Plans of Care in collaboration with physicians and the interdisciplinary team.
• Provide skilled nursing interventions, such as wound care, IV therapy, medication administration, and patient/family education.
• Monitor patient progress, update documentation, and adjust care plans as needed.
• Maintain accurate, timely, and compliant clinical documentation in the EMR system.
• Communicate effectively with physicians, case managers, therapists, and other healthcare professionals to ensure coordinated care.
• Educate patients and families on disease management, safety, and home health best practices.
• Uphold all regulatory, accreditation, and agency standards.

Qualifications
• Current, active Registered Nurse (RN) license in the state of California.
• Minimum of 1 year of experience in a home health agency.
• Strong knowledge of Medicare, OASIS documentation, and home health regulations.
• Proficiency in wound care, patient assessments, and care planning.
• Excellent communication, organizational, and critical thinking skills.
• Ability to work independently in the field and manage time effectively.
• Reliable transportation and valid driver’s license.
• Current CPR/BLS certification.

• $120–$140 per visit for SOC, Recertification, and Discharge.
