Divine Home Health Care is looking for Skilled Nursing (RN/LPN) and CNA care. Job would need someone experienced in caring for a complex medical needs client in their own home. Hours needed are Fri/Sat/Mon day shift, 6 am - 6 pm. Prefer skilled nursing for those hours but will use Certified Nursing Assistant as well. CNA would need to have completed both Nurse Delegation-Core and Nurse Delegation-Diabetes classes to be able to be delegated under the supervision of the RN. RN/LPN duties would include med management, wound care, trach care, other care duties, and supervision of the CNA. RN/LPN would have CNA assistance for 6 hours of the day shift. CNA positions are Fri, Sat, Mon - 7 a.m.- 1 p.m. Tuesday NOC shift - 6 p.m. - 6 a.m. and on-call for nights in general. IHSS Provider in home caregiver needed for a Disabled 54 y/o woman, wheelchair user. Provider must wear well fitting mask at all times on duty and must be completely fragrance free in all body and hair products, especially laundry detergent. No vape or smoking. Split 176 hours with main provider. Approximately 72 hours a month. Tue, Thur, Fri half day until 5:30 p.m. (start time varies but same each month) or potential 8-hour shifts Thursday and Friday 9:30 a.m. to 5:30 p.m. (occasional Tuesday or alteration). Potential for increased hours after December. Duties include: Assistance with personal hygiene, including bathing, dressing, and grooming. Medication reminders and management. Meal preparation and housekeeping. Meal cleanup, laundry, grocery shopping , errands. Safe transfers and mobility assistance. Accompaniment to and assistance with medical appointments. CPAP cleaning.
