In this role, you will use your clinical expertise to conduct Start of Care (SOC) assessments, collaborate closely with physicians, and administer critical treatments including IV therapies. Accountabilities: Perform Start of Care (SOC) admissions and complete OASIS documentation for new patients. Conduct follow-ups and evaluation visits on weekends in the assigned territory. Monitor and document patient conditions and coordinate care plans with physicians and internal teams. Administer IV medications such as antibiotics and diuretics in accordance with protocols. Provide hands-on patient care including wound treatment and medication reconciliation. Maintain accurate and timely documentation of all care interventions and outcomes. Collaborate with an interdisciplinary team to ensure the highest standards of care delivery. Requirements Active and unrestricted Registered Nurse license in California. Current BLS/CPR certification and valid CA driver’s license. Experience with Start of Care (SOC) and OASIS documentation preferred. Background in high-acuity patient care and case management is strongly desired. Proficiency in IV therapy initiation and medication administration is a plus. Strong critical thinking, organizational, and interpersonal communication skills. Self-directed, empathetic, and collaborative team player with leadership potential.
