We are seeking a strong and COMPASSIONATE Administrative Case Assistant to join our team in the Mill Basin/Marine Park section of Brooklyn. The ideal candidate will support a 20-25 team of consisting of social workers whom provide case management to frail elderly clients in Brooklyn Full time M-F 9-5. Hybrid schedule 20% remote work, 80% in office. Organization is 100 years old and a staple in the community. Ensure clients receive meals on wheels and other related services * Identify and use appropriate resources to respond to participant needs and send any referrals necessary ( directed by Social Work staff) to implement care plans. Provide follow-up with clients and collateral contacts as indicated. * Input data and maintain comprehensive electronic and paper case files, including intakes, care plans, case progress notes, event participation, and units of service provided, and other important documents. * Liaise with partners and community groups, attend fairs and client events in neighborhood. * Strong follow up and organizational skills * Responsible for ordering supplies, managing office needs, answering live phone, filing, copying, scheduling appointments * Access outside resources and/or programs to facilitate all unmet needs of clients * Assistant to the Executive Director * Communicate client updates to all relevant parties * Facilitate referrals to other healthcare professionals and programs * Maintain accurate client documentation Qualifications: * Associate in Arts or HS Diploma with demonstrated paid experience * Experience in human services or social workers a plus * Demonstrated experience in client relations and administrative tasks • Demonstrated experience with aging population, preferred. • Understanding of the aging process and its impact on individuals physically, socially, mentally, etc. • Demonstrated experience with working as a member of an interdisciplinary team. • Experience with volunteer activities a plus. • Excellent organizational, verbal, written, and interpersonal skills. • Strong problem solving skills with the ability to effectively handle crisis situations • Excellent computer skills.
