This position will perform required case management services, maintaining client caseloads consisting of individual with varying degrees of disabling conditions to assist them in achieving a competitive employment outcome. The program manager will coordinate care in a client-centered manner, focusing on individual strengths, and the needs of the client, and conducting vocational evaluations to assist the client in job placement. Later Years is a private care home that provides care to 6 elderly residents. We are looking to hire someone for 2 - 24 hour shifts. This position is an overnight position that allows sleep time.
