Tiburcio Vasquez Health Center is a non-profit community health center that is dedicated to promoting the health and well-being of our community by providing accessible, high-quality care. We integrate primary care, dental care, WIC support, mental health counseling, community health education, and more. Under the general direction of the Nursing Manager, performs Registered Nursing (RN) care, including triage of patient complaints via Telemedicine (Video/phone consultation) and/or in-person services. The RN will promote and restore patients’ health by providing, delegating, evaluating, and coordinating comprehensive professional nursing care for all patients As a patient advocate, the RN will also provide whole person support to patients, their friends, and families. The RN may oversee clinical care as compatible with their licensure, including monitoring of duties performed by LVN and MA staff. This is a full-time position, working 40 hours per week, typically Monday through Friday with periodic Saturday hours. Compensation: $88,753.60 - $96,990.40 annualized, depending on experience This position is represented by SEIU, with compensation and benefits determined by the terms of the Collective Bargaining Agreement (CBA). Salary is based on the wage scales outlined in the agreement. Responsibilities: Works independently and as part of an interdisciplinary care team to provide nursing services, including triage of health complaints, coordination of care, agenda setting, referrals to other providers and community health resources, health education, and provision of direct patient care services (e.g., immunization, medication administration). Coordinates care with Clinical Support staff (Licensed Nursing, Medical Assistants, Clinic Operations leadership) at clinical site if in-person services are indicated (e.g., point of care testing) in the provision of triage nursing care. Performs telephonic triage for patients contacting the organization seeking medical care. Provide telephonic responses to patient’s questions regarding patient’s medical care or treatment. Performs assessment data collection and documents subjective data (“Chief Complaint”). Reviews basic health history information for patients, focusing on environmental, preventive, and self-care planning. Collaborates with the physicians and advanced practice providers (APPs)and the rest of the care team to meet patient care needs. Perform in-person triage for unscheduled patients arriving at the clinic as needed. Performs assessment data collection and documents subjective data (ie “chief complaint, history of present illness, medication review, etc) and objective data (e.g., vital signs/weight). Reviews basic health history information for patients, focusing on environmental, preventive, and self-care planning. Collaborates with the physicians, APPs and care team to meet patient care needs. RN conducts co-visits with the physicians and APPs with in-clinic examination and procedures as appropriate. Elicits and records the history of present illness and past medical, family, and social histories. Collects vital signs, implements pertinent standing orders. Follows protocols, RN reviews the care plan and provides appropriate education to the patient during visit. Performs interpretation and applicable documentation of Mantoux Tuberculin skin tests. Performs the appropriate documentation of care delivered. Review and address patient needs identified from After Hours nursing notifications, including triage services and chart updates. Supports the coordination of care across services, settings, and teams as a member of the care team. Educates patients and families on disease processes, medications, lifestyle changes, and self-care techniques. Participates in quality improvement, population health, and practice transformation initiatives. Provides immunizations, wound care, medication administration, and other procedures as appropriate to licensure. Responds to patient inquiries and follow-up needs in person, by phone, or via secure messaging. Acts as a resource for members of the team Attends meetings as requested. Assists leadership on matters relating to nursing practices, and participates in development of policies and protocols for patient care. Participates in orientation, training, growth and development of new hire and staff. Evaluates the effectiveness of the care given by interdisciplinary care team and identifies problems making timely recommendations for their solution. Performs all duties and services in full compliance with TVHC’s Service Excellence Standards. Performs all duties necessary for successful EHR/EPM (EPIC) entries and maintenance for patients/clients within designated time set by the organization. Performs other related duties as assigned Requirements Valid license to practice as a Registered Nurse in the state of California. Graduate of an accredited Nursing program required. Experience in an ambulatory care environment preferred. Experience in working with patients around chronic condition management and preventive care services BLS Certification required. Qualifications: Excellent communication, critical thinking, and teamwork skills Ability to deliver outstanding patient experience Ability to demonstrate proficiency in clinical skills assessment in each specialty - Adult Medicine, Family Medicine, Women’s Health, and Pediatrics Ability to use essential software and applications associated with the role’s duties and responsibilities. Bilingual in English/Spanish or language(s) strongly preferred. Demonstrates organizational and time management skills, with the ability to prioritize multiple projects while delivering quality service/achieving business results Possess a thorough understanding of the importance of confidentiality and non-disclosure according to the general standards set forth by HIPAA. Culturally sensitive and demonstrated ability and effectiveness working with ethnically diverse populations. Demonstrate flexibility regarding job duties and assignments. Ability to travel to various clinics, as required.
