 Position Title: Youth Case Manager-Electronic Monitoring Position Type: Full Time; 35 Hours Weekly Benefits Eligible: Yes; Full Benefits Location: Toms River, NJ Department: Children & Families Salary: $40,000-43,000 Responsibilities: Participate in Ocean County family court proceedings and multi-disciplinary treatment team weekly and annual Youth Services Commission program monitoring. Engage all youth consumers via initial assessment, weekly home visits, Youth Center weekly activities, and referral and linkage in the community. Create electronic updates to court on youth services provided. Assist youth and families with resources and obtaining basic needs through Family Court Navigation. Provide ongoing case management and monitoring via GPS ankle bracelet when ordered by court. Respond to GPS alerts when needed. Assist Family Crisis Intervention Unit (FCIU)/Family Navigator as a team approach Provide psychosocial education and transportation for youth and family members as needed. Provide crisis intervention as necessary Rotating on-call duties as assigned.
