Social Worker (LSW) – Full-Time with Weekend On-Call – Nursing Home Title: Licensed Social Worker (LSW) – Full-Time, Monday–Friday (weekend on-call as needed) Location: Long-Term Care / Nursing Home (Location Flexible) Compensation: $60–$70 per hour Pay delivered weekly via direct deposit Includes health insurance and additional benefits What You’ll Be Doing As our Social Worker, you’ll be a vital advocate for residents—blending clinical expertise with compassion to elevate quality of care: Conduct thorough psychosocial assessments, develop and implement care plans, and support discharge planning. Act as an empathetic liaison for residents and families, ensuring dignity, involvement, and clear communication around care decisions. Respond to on-call needs during weekends, managing urgent support or crisis situations as required. Collaborate with interdisciplinary teams—nurses, therapists, admission staff—to deliver integrated, person-centered care. Connect with community resources, coordinate external services, and streamline transitions between care settings.
