Job Summary: The Family Nurse Practitioner (FNP) provides adolescent-focused family and integrated medical care with an emphasis on medication management, accurate documentation, collaborative communication with families, and interdisciplinary coordination. This position blends remote telehealth services with occasional onsite visits based on patient need and supports evidence-based, outcomes-oriented treatment. The typical caseload includes 3–4 intakes per week plus ongoing case management. Job Duties & Responsibilities: Conduct comprehensive medical evaluations and provide ongoing medication management for adolescent patients using age-appropriate, evidence-based practices. Order and interpret lab work and screenings (e.g., UDS, pregnancy tests, CLIA-waived tests); ensure follow-up and clinical integration. Complete medication reconciliations at admission, transition, and discharge. Provide clear education on medications, obtain informed consent, and support shared decision-making with patients and families. Incorporate pharmacogenetic or other clinical testing when appropriate into care planning. Document all patient encounters accurately and on time in the EMR, closing all notes, orders, and assessments per policy timelines. Respond promptly to Utilization Review and billing requests to support timely claims processing. Maintain accurate medication records and close all orders/tasks quickly to ensure compliance. Serve as the consistent contact for parents/guardians regarding diagnoses and medications; address concerns promptly and with compassion. Attend treatment team meetings, collaborate with therapists, case managers, and neuropsychology providers to coordinate and adjust care plans. Maintain licensure, certification, and practice within legal and ethical boundaries per Arizona and federal regulations. Support crisis planning, de-escalation, and safety protocols as needed. Maintain punctuality for all appointments and meetings, and communicate schedule changes in accordance with clinic expectations. Typical caseload includes 3–4 intakes per week and general case management for ongoing patients. This is primarily a remote role, but some in-person clinic travel may be required based on patient or program needs. Prerequisites / License & Certification Requirements: Active, unrestricted Arizona Nurse Practitioner license NP program must be completed in the United States (not Puerto Rico) Current CPR/BLS certification Active Level One Fingerprint Clearance Card Valid driver’s license and ability to pass background checks Adolescent behavioral health or psychiatric experience (preferred but not required) Collaborative approach to interdisciplinary and neuropsychology-integrated care If you meet all of our criteria and would like to be considered, please apply with your most updated Resume/CV. Prior experience with assessing feasibility/cost and detailed design for excavation and off-site disposal projects, in-situ chemical injections (oxidant and reductant approaches), sub-slab depressurization systems, and other remedial systems is required. The successful candidate will work with junior engineers, geologists and environmental scientists to design and manage field work including groundwater, soil, soil vapor, and surface water sampling, manage and evaluate field and lab data, supervise contractors and drillers, and support the preparation of reports and conduct review of reports developed by junior staff.
