Description

Case Manager for Behavioral Health Program Responsibilities of the Case Manager:  The Case Manager needs to demonstrate initiative, confidence, and a focus on making positive connections to coordinate care management services for the Behavioral Health Home clients. Duties include: Integrating, overseeing and monitoring all aspects of the physical and behavioral health needs of Behavioral Health Home clients. Complete comprehensive assessments, develop and monitor interdisciplinary plan of care aimed at achieving stability within the community. Provide referrals and linkages to community services. Regularly communicate with other care providers to exchange information and identify needs. Provide outreach to those that are in need but not currently receive service and coordinate with managed care companies.  Provide transportation to clients using the agency’s vehicles. Promote a culture of inclusion and belonging. Requirements Skills/Qualifications for the Case Manager include: BA/BS in human services. Minimum of 2 years human services. 1 year in case management (defined as experience in assessment of client’s need, care planning, implementation of care plan and regular review along with benefit entitlements, linkage to community providers and services). A demonstrated ability to function as a member of an interdisciplinary team. Knowledge of mental illness/behavioral health, substance abuse, physical/medical health, and homelessness is required. 1 year experience working in a health care setting providing direct care to clients is preferred. Valid and insurable driver's license. Agency vehicles are available for transporting clients.  Must have a personal vehicle to use for business purposes when not transporting clients.  Benefits Salary and Schedule of the Case Manager: $24.46/hour. $50,876.80/year. (delete OT rate) 40 hours per week.  Monday-Friday.  DAY TIME HOURS AND NO ON-CALL. Some flexibility with early morning hours as clients might have early morning medical appointments.
