About the Role: As a Care Coordinator, you will be responsible for managing day-to-day care staffing for clients in both our In-Home (Kansas) and HCY (Missouri) programs. You’ll work closely with families, caregivers, and clinical teams to ensure quality care, timely communication, and continuity of services. What You'll Do: Coordinate caregiver schedules based on client needs and caregiver qualifications Build meaningful relationships with families and caregivers to ensure trust and care satisfaction Maintain accurate documentation and reports per program guidelines Communicate clearly and professionally across departments and with external partners Assist with onboarding and training new caregivers to ensure a high standard of care Respond to staffing needs quickly and creatively to ensure client support Represent Phoenix with professionalism and empathy at all times What You Bring: A true passion for working with children with special needs and the elderly Experience in home care, case coordination, scheduling, or human services preferred High school diploma or equivalent required Ability to multitask, problem-solve, and adapt quickly in a fast-paced environment Strong communication and organizational skills Heart-centered, client-first mindset Why Phoenix?
