The Panel Specialist, Medical Assistant plays a vital role in managing patient care and supporting efficient clinic operations. Working closely with healthcare providers and medical staff, the Panel Specialist delivers high-quality patient care through both administrative and clinical responsibilities. These include patient coordination, record management, and direct patient interactions. This position must exemplify the core values and mission of the organization and consistently demonstrate discretion, diplomacy, and tact in patient and staff interactions. This is a full-time position working 40 hours per week, typically Monday through Friday with periodic Saturday hours. Tiburcio Vasquez Health Center is a non-profit community health center that is dedicated to promoting the health and well-being of our community by providing accessible, high-quality care by integrating primary care, dental care, WIC support, mental health counseling, community health education, and more. Compensation: $24.21 - $26.45 per hour, depending on years of experience. This position is represented by SEIU, with compensation and benefits determined by the terms of the Collective Bargaining Agreement (CBA). Salary is based on the wage scales outlined in the agreement. Beyond base pay, TVHC provides a comprehensive compensation package that supports the health, wealth, and career development of our employees. Responsibilities: Reports directly to the Clinical Support Services Supervisor with matrix reporting to the Clinical Team Supervisor and clinical supervision by provider(s). Collaborates with an interdisciplinary care team to provide proactive healthcare management for an assigned patient panel. Regularly runs quality data dashboards to identify patients needing follow-up care and facilitates outreach through MyChart or phone. Coordinates preventive care using standing orders (e.g., cancer screenings, immunizations, lab work). Addresses incoming MyChart messages and manages patient care requests. Processing of and any resulting follow-up required for laboratory services and procedures completed by outside services not electronically interfaced with EHR. Processing or and any resulting follow-up required for non-formulary medications prescribed by Healthcare Providers. Submits supplemental documentation to insurance providers for dispense of non-formulary prescriptions by pharmacy. Management of patient care requests received on MyChart application. Prepare provider forms (paper and online delegation) for Healthcare Provider signature based on patient electronic health information as appropriate to scope. Performs patient chart updates during pre-visit prep, including reconciliation of medication list; including name(s) of meds, dosages, frequency, duration and start date. Utilizes standing orders to provide patient care as directed. Obtains and reviews medical reports from external providers and facilitates necessary follow-up. Administers clinical quality improvement projects, tracks diagnostic results, and provides patient navigation support. Provides basic clinical services as needed including, but not limited to administration of medications and injections to patients as directed by the provider; immunizations; referrals to other providers and other health resources; and health instruction to patients of following examinations. Performs all duties necessary for successful EHR (EPIC) entries and maintenance for patients/clients within the designated time set by the organization. Able to adapt quickly to new EPIC workflows. Provides coverage for other Panel Specialists as needed. Attends general and medical staff meetings. Requirements High school diploma (or GED), or trade certificate is required. Clinical Medical Assistant certification from an accredited institution is required. Minimum of three (3) years of recent experience as a Medical Assistant is required. BLS certification required. Bilingual in English/Spanish strongly preferred. Familiarity with community/service area preferred. Experience working with data systems used in a medical setting preferred. Qualifications: Excellent communication skills at the level necessary for patients’ outreach and in reach to close care gaps, understanding provider and supervisor instructions, and accurately documenting patients’ medical information. Ability to effectively communicate with patient population and staff while demonstrating a high degree of diplomacy and tact. Intermediate to advanced computing and phone skills. Able to receive and understand ongoing training on report building and data stewardship Working knowledge of “Universal Precautions” and demonstrated professionalism at all times. Ability to multi-task and work effectively in a high-stress and fast-moving environment Possess a thorough understanding of the importance of confidentiality and non-disclosure according to the general standards set forth by HIPAA. Culturally sensitive and demonstrated ability and effectiveness working with ethnically diverse populations. Demonstrated flexibility and willingness to work on an as-needed basis in order to meet the operational needs of the clinic. Must have the ability to travel to and from various service sites as directed.
