For more information about our company, please visit us online at Supportinc.com HOST HOME PROVIDER POSITION We are seeking experienced Host Home Providers who are willing to open their home and foster an individual who can no longer live independently in their own community and don’t want to live in a communal setting. Providers share their home and lives with individuals with Developmental Disabilities.  As a Host Home Provider, you take the client into your home and provide them with a private bedroom and hands-on support and ongoing daily care.  You can help someone who has significant medical concerns or behavioral challenges and make a difference in their life as they become part of your family. HHP Provider Duties and Responsibilities: Provide personal care including assisting with hygiene care, meal preparation, dressing Provide employment coaching to assist in getting a job Provide a variety of recreational activities to enhance social development Include client in family life and activities Accompany client to supermarket, museums, parks, dances, recreation centers, etc. Provide transportation as needed Assist with medical care, including appointments and administering medications Provide documentation of progress notes and medical records Provide training on life learning skills To be successful, candidates . . . Should have a high school or equivalent education (Required) Must complete and pass criminal and motor vehicle background checks Must complete training for Medication Administration, CPR/First Aid and maintain ongoing training requirements Must have at least 1 year of experience as a host home provider Should preferably be experienced with behavioral needs Should preferably be experience with medical needs, preferred (i.e. CNAs, LPNs, RNs, etc.) Support Inc. Yoakum Community Hospital is seeking a Medical Assistant to join our team in the Family Practice Clinic. The ideal candidate will perform essential duties under the close supervision of a Physician, Nurse Practitioner, or Lead Nurse, and will be accountable for all patient care provided during their shift. This role requires adherence to all Standards of Nursing Care and a strong commitment to promoting teamwork. Key responsibilities may include: Rooming patients, taking vital signs, and accurately documenting patient information in the electronic medical record. Administering injections and routine office treatments such as nebulizer, ear lavage, and peak flow. Assisting with patient flow and scheduling diagnostic procedures, clinic appointments, and hospital procedures. Handling phone calls, including screening calls from patients for the provider and calling in new or refilling medications. Ensuring exam rooms are clean, orderly, and well-stocked. Maintaining patient confidentiality in accordance with HIPAA regulations. Requirements 3 to 5 years of previous medical assistant experience in an office-based practice is required. A High School Diploma or GED is required. Medical Assistant Certification is preferred. Qualifications: Genuine interest in the welfare of the ill. Excellent verbal and written communication skills. Ability to read and interpret medical documents, including ICD-9 Manuals and physician orders. Ability to solve practical problems and define issues, collect data, and draw valid conclusions. Proficiency in basic arithmetic, including fractions, percentages, and ratios.
