We are seeking an experienced and compassionate Hospice Registered Nurse (PRN basis) to join our dynamic team in promoting and practicing the Jumpstart Hospice and Palliative Care’s mission “to enrich the final stages of life with deep compassion and understanding.” This is a flexible, on-call position perfect for nurses who want to make a meaningful difference while maintaining a work-life balance. Key Responsibilities: - Provide skilled nursing care to hospice patients in accordance with the care plan. - Educate and support patients and families about end-of-life care. - Communicate with interdisciplinary team members, including physicians, chaplains, social workers, and aides. - Document patient visits and updates accurately and promptly. - Respond to on-call needs including but not limited to crisis visits, symptom management, and admissions. - Maintain up-to-date and accurate EMRs, including care plans, IDG notes, and clinical documentation - Serve as the primary liaison between the patient, representative and hospice team - Adhere to professional standards as outlined by protocols, rules and regulations - Performs other duties as assigned Qualifications: - Active RN license in Georgia - Strong clinical and assessment skills - Excellent communication and interpersonal skills - Ability to work independently while being a collaborative team player - Valid driver’s license and reliable transportation.
