Community Facilitators The duties of CF's provides vocational and/or skills training to persons with developmental disabilities on a 1:1 basis. Duties include: - Follow Clients' Person-Centered Planning - Assist client in selecting realistic goals and objectives - Assist clients in participation in chosen club activities - Assess, evaluate, implement and chart client's progress on goals and objectives - Adhere to external employment site-rules, regulations and scheduled work hours - Record data and maintain field books - Follow all policies and procedures as they relate to job performance - Provide constant supervision to clients - Train and monitor clients in site specific safety procedures including infection control - Accept changes in assigned clients or program sites as directed - All other duties as assigned by the Program Manager - Mandated Abuse Reporter ****DSP's encouraged to apply***** ****Bilingual in Spanish and English****
