Home Health Care Agency is seeking part time Registered Nurses with/without Home Health experience. Patient service area is Portland/surrounding areas. Responsibilities and Duties: • Perform evaluation tasks, including vital signs and medication review and complete SOC • Develops a Plan of Care based on the patient diagnosis and needs. • Dress or redress open wounds and assess progress of healing • Educate patients and families on home health care strategies and procedures • Make recommendations for devices or tools that might improve the patient’s quality of life • Coordinate with Occupational Therapists, Physical Therapists, Physicians and others involved in the patient’s care plan. • Participates in Case Conference, in-service and meetings • Coordinates and establishes programs geared towards patient care • Maintains all communication amongst field staff and office staff Qualifications and Skills: • Registered Nurse in the State of OR • Home Health Experience is a Plus • Excellent communication and time-management skills • Compassionate and friendly demeanor • Ability to travel (locally) with reliable transportation Reply to this email or Fax your resume to 503-512-5384 to schedule zoom meeting
