ROLE SUMMARY: Under the direct supervision of the Supervising Nurse, an employee of this class is responsible for providing direct nursing care to the program individuals. The licensed practical nurse is responsible for the assessment of health status, development of the plan of care, implementation of treatment plan and evaluation of patient status, evaluation of individuals for changing levels of care, and collaborating with the health care team to concentrate on preventive services and maintain a person-centered approach. Licensed practical nurses will educate, certify and supervise AMAP (approved medication administration person) staff in the performance of nursing tasks and activities.
