Now Hiring: Nurse Practitioner (NP) / Physician Assistant (PA) – Post-Acute Care (Skilled Nursing Facilities) Locations: Mesa, Gilbert, and Chandler, Arizona Schedule: Part-Time | Weekdays | Weekend once a month Practice Setting: Skilled Nursing Facilities (Post-Acute / Long-Term Care) Compensation: Full-time: $80-$95/hour | Part-time: Salary will be proportionate based on hours worked/Patients seen Employment Type: 1099 Independent Contractor About the Role We are seeking a Nurse Practitioner or Physician Assistant to provide direct in-person care at skilled nursing and post-acute care facilities across Mesa, Gilbert, and Chandler, AZ. This role is ideal for a provider seeking flexibility and autonomy while making a meaningful impact on patient outcomes in long-term care settings. As an independent contractor, you will play a vital role in maintaining continuity of care and reducing unnecessary hospitalizations, ensuring patients receive high-quality treatment throughout the weekend. Key Responsibilities Complete weekend rounds across multiple skilled nursing facilities Perform patient evaluations and develop individualized care plans Manage acute conditions and chronic disease exacerbations Order, review, and act on laboratory, imaging, and diagnostic results Adjust medications and therapies as clinically appropriate Collaborate closely with facility staff, physicians, and interdisciplinary teams Educate patients, families, and facility staff regarding treatment goals and expectations Document all clinical encounters promptly and accurately in the EMR Support discharge planning and smooth care transitions Ensure compliance with SNF protocols and regulatory requirements Qualifications Active, unrestricted Arizona license as a Nurse Practitioner or Physician Assistant Graduate of an accredited U.S.-based NP or PA program (Puerto Rico-based programs not eligible) Preferred: 1–2 years of experience in post-acute care, long-term care, or skilled nursing facilities Proficiency with EMR systems and post-acute documentation standards Excellent clinical judgment, communication, and organizational skills Ability to work independently within clearly defined clinical parameters Why You’ll Love This Role Competitive hourly compensation with steady part-time hours Opportunity for independent practice within a supportive care network Flexible supplemental position for experienced providers Impactful work improving health outcomes in high-need populations How to Apply If you are a compassionate and experienced NP or PA interested in providing quality weekend coverage in skilled nursing facilities, we invite you to apply.
