You’re team-oriented, proactive in your clinical approach, and comfortable managing patients across the lifespan. Experience in family medicine is preferred, though highly qualified new graduates will be considered. Key Responsibilities Perform comprehensive and focused visits, including annual physicals and preventive screenings Diagnose and manage chronic illnesses such as diabetes, hypertension, asthma, and COPD Order and interpret diagnostic tests and imaging studies Develop and implement individualized treatment plans Provide education on disease prevention and healthy lifestyle practices Collaborate with physicians, RNs, and administrative staff Document all clinical encounters in the EHR with scribe support Manage a patient panel of approximately 10–15 patients per day to ensure quality outcomes Required Qualifications Active, unrestricted Arizona license as a Nurse Practitioner or Physician Assistant Graduate of an accredited NP or PA program Current DEA registration Eligible for credentialing with Medicare, Medicaid, and commercial insurers Clean professional record with no history of malpractice or disciplinary actions How to Apply If you’re looking to join a mission-aligned organization that prioritizes quality, accessibility, and team culture, we encourage you to apply.
