Position Overview We are seeking a compassionate and motivated Nurse Practitioner (NP) to provide telehealth consultations and in-home visits for senior patients. This role is ideal for a new graduate looking to gain experience in geriatric care while working in a flexible and supportive environment. Key Responsibilities - Conduct telehealth assessments and consultations for senior patients. - Perform in-home visits to assess, diagnose, and treat patients in their living environments. - Manage acute and chronic conditions with a focus on geriatric primary care. - Develop and implement personalized treatment plans to improve patient health outcomes. - Provide patient and caregiver education on chronic disease management, medication adherence, and preventative care. - Coordinate with physicians, specialists, and other healthcare providers to ensure comprehensive care. - Document all patient interactions in an electronic health record (EHR) system. - Recommend and facilitate preventative screenings and wellness programs. - Participate in team meetings and case discussions to optimize patient care strategies. Requirements - Graduate of an accredited Nurse Practitioner (NP) program (Master’s or Doctorate in Nursing). - Board-certified as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP). - Active and unrestricted NP license in California. - DEA license or eligibility to obtain one. - CPR/BLS certification. - Passion for geriatric care, chronic disease management, and telehealth services. - Comfortable working independently and traveling throughout Los Angeles County for patient visits. - Proficiency in telehealth platforms and electronic health records (EHR) (training provided if needed). - Strong communication and interpersonal skills, with a commitment to providing compassionate patient care. - Full-Time | Remote & Home Visits Benefits - Competitive salary ($120,000 - $150,000 based on experience). - Flexible schedule – blend of remote telehealth and home visits. - Mileage reimbursement for travel to patient homes. - Comprehensive benefits package (health, dental, vision, 401k, and malpractice insurance). - Paid time off (PTO) and continuing medical education (CME) allowance. - Opportunity to grow within a leading telehealth agency specializing in senior care.
