Description

We are looking for someone who can provide outstanding patient care and physical support on a daily basis. A Certified Nursing Assistant (CNA) will provide compassionate care and assistance with many daily living needs for patients. The ideal candidate will spend time interacting directly with patients, monitoring their vital signs, and assisting when necessary. The ideal candidate will relate well as an advocate for patients through conversation and interaction to improve the patient’s experience while at our facilities. Responsibilities Assist with daily living activities to make sure the patient is taken care of. This includes turning and ambulating patients and delivering food and beverages to patients throughout the day Help patients with personal hygiene, such as providing bedpans and helping them bathe Provide adjunct patient care as needed. This can include delivering ice packs and dressings to help relieve pain while in therapy or baths for soothing purposes after their session has ended Closely monitor and record food intake, as well as urinary and fecal output, to be shared with key medical and nursing staff Requirements State Certified Nursing Assistant Certification Current CPR Certification (American Heart Association) Basic computer skills The ability to think and work independently, with direction from a team leader or manager, while maintaining efficient and consistent communications at all times Must also have excellent oral communication skills when speaking with other health professionals in the presence of patients (i.e. “bedside manner”) Benefits At Masonic Homes, we are looking for team members who would like to combine their passion for helping people, good communication skills, decision-making skills, and great ethical standards with the opportunities to advance your career. Position Overview We are seeking a compassionate and motivated Nurse Practitioner (NP) to provide telehealth consultations and in-home visits for senior patients. This role is ideal for a new graduate looking to gain experience in geriatric care while working in a flexible and supportive environment. Key Responsibilities - Conduct telehealth assessments and consultations for senior patients. - Perform in-home visits to assess, diagnose, and treat patients in their living environments. - Manage acute and chronic conditions with a focus on geriatric primary care. - Develop and implement personalized treatment plans to improve patient health outcomes. - Provide patient and caregiver education on chronic disease management, medication adherence, and preventative care. - Coordinate with physicians, specialists, and other healthcare providers to ensure comprehensive care. - Document all patient interactions in an electronic health record (EHR) system. - Recommend and facilitate preventative screenings and wellness programs. - Participate in team meetings and case discussions to optimize patient care strategies.
