Registered Nurse - Primary Care RN

We are seeking an enthusiastic and dedicated Registered Nurse (RN) for our Primary Care team. In this role, you will provide exceptional nursing care to patients in a primary care setting, focusing on health education, patient advocacy, and the management of chronic and acute illnesses. You will work closely with a multidisciplinary team to deliver patient-centered care to improve overall health outcomes. Key Responsibilities: Conduct thorough patient assessments and triage patients efficiently. Develop and implement individualized care plans in collaboration with the healthcare team. Provide health education to patients and their families regarding chronic disease management, medication adherence, and preventive care. Perform routine nursing procedures such as administering medications, injections, and immunizations, as well as assisting with minor procedures. Monitor and document patient responses and progress in electronic health records. Coordinate follow-up appointments, referrals, and community resources for patients as needed. Participate in quality improvement initiatives to enhance patient care standards. Work collaboratively to ensure continuity of care and promote a healthy lifestyle among patients. Requirements Active and unrestricted RN license in the applicable state. Associate's Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN). 3+ years of experience in a primary care or outpatient clinic setting preferred. Current CPR/BLS certification (AHA preferred). Strong communication, organizational, and patient advocacy skills. Ability to work in a team-oriented environment and adapt to a dynamic healthcare setting. Proficiency in using electronic health record systems and technology for patient management. Must pass a government background check. Able to provide 3 professional references.
