The LPN Caregiver provides the patient with personal care assistance and home support in an effort to maintain a clean, safe environment for the elderly or convalescing patient thus allowing them to remain in their own home. LPN Caregivers work in patient homes and in facilities. The LPN Caregiver possess the ability and willingness to interact with residents, family members, personnel, etc., under all conditions and circumstances. Executes the patient care plan and communicates well with RN Case Manager. Monitoring the physical, mental, and emotional conditions of clients and reporting changes to designated parties. Documents care using Agency approved Electronic Visit Verification system from the patient's care location. Consistently shows accountability to schedule and communicates all needs for scheduling changes to Staffing Coordinator. With prior authorization and proper training, administering medicine and treatments as prescribed by healthcare providers. Requirements Ability to read, write, and communicate effectively Ability to understand both written and verbal directions and instructions Graduate of an accredited LPN program Current LPN license (PA) Successful completion of a job specific knowledge exam Possession of strong organization skills Understanding, compassionate attitude toward the care of the sick Ability to document actions and observations with detail and accuracy One year home health experience preferred. Familiarity with Feeding Tubes, Assisted Breathing Devices (Ventilators) and Hoyer Lift Proof of COVID-19 Vaccination or an approved Medical or Religious exemption.
