ROLE HIGHLIGHTS - What to expect as a Regional Nurse Home Care Case Manager
As Regional Nurse (RN) Home Care Case Manager, you oversee the nursing standards for the Caring for Family of Companies. You're responsible for the highest quality of case management and care for Clients – as well as providing high caliber Caregiver training and professional development. This position reports to the Director of Nursing Services, and collaborates with an entire team of Nurses for ongoing support
Nursing Program Oversight
Oversee nursing services, including medication administration program
Ensure standardization and adherence of process
Maintain strict compliance with established protocols and the In Home Care licensing standards
Review orders and reports to maintain safety and compliance protocols
Collaborate with community providers
Client Evaluation
Conduct nursing and medication evaluation and assessment visits, addressing Client concerns
Provider Training
Train Caregivers on policies and medication administration, evaluating competency
Train Caregivers on Nurse-delegated duties, ensuring competency and ability to deliver care
Multi Site Nursing Support
Travel to regional branches to fill RN role gaps and support as needed

Next Level Professionalism
Experience handling highly confidential and sensitive information. - and/or -knowledge of best practices for handling PHI as governed by HIPAA

Excellent written and oral communication and fluency in English

Tech Savvy
Proficient in Microsoft Suite (Word, Excel, Sharepoint, Outlook)
Proficient with technology, software, and electronic devices
EDUCATION AND SPECIALIZED EXPERIENCE REQUIRED:
Specialized experience with the following:
EHR or Care management platforms (ex: Epic, Wellsky, AxisCare, Alayacare)
Educational requirements:
RN License in the state of Oregon
