What You’ll Be Doing: You’ll be a consistent, compassionate presence during a critical time of day for our clients and their families. Responsibilities may include: Administering evening medications and treatments Providing g-tube, trach, or ventilator support Monitoring vital signs and general condition Documenting care provided and changes in condition via electronic charting Communicating with families and care team members as needed Following the physician’s plan of care with attention and compassion You’ll work independently in the client’s home—but never without support.
