As a Home Health Case Manager, you will play a vital role in delivering hands-on patient care right in the comfort of their own homes. Your expertise will be needed to coordinate the services of other healthcare providers, communicate with health insurance companies, and oversee the work of visiting nurses. A strong background in physical assessment, diagnostic interpretation, and effective communication will be essential in developing a personalized care plan that aligns with physician orders and incorporates thorough initial assessments. You will be collaborating closely with fellow nurses, as well as professionals in Physical Therapy, Speech Therapy, and Occupational Therapy. In this role, you will be responsible for managing the schedules of our caregivers and ensuring our clients receive timely and appropriate care. Your exceptional organizational and communication skills will contribute to the overall efficiency of our home care services. Salary: $22-24/hr, based on experience. Hours: M-F, on-site, 9am-5:30pm Responsibilities Schedule and coordinate assignments for field nurses. Manage clinical updates and workflows including: Hospitalizations, holds, discharges, and authorization requests Serve as the point of contact for all patient-related clinical matters. Support the clinical team with timely communication and accurate record-keeping. Ensure all required clinical documentation is completed and filed appropriately. Collaborate with care managers, intake staff, and other internal departments to ensure seamless patient care transitions. Requirements Prior experience in a clinical coordinator, nursing scheduler, or home care administrative support role preferred. Understanding of home care policies and clinical procedures (hospitalizations, authorizations, etc.). Strong organizational skills with the ability to manage priorities in a fast-paced environment. Excellent verbal and written communication skills. Proficiency with scheduling software and EMR systems is a plus. Bilingual (English/Spanish or other languages) is a plus.
