This position will perform required case management services, maintaining client caseloads consisting of individual with varying degrees of disabling conditions to assist them in achieving a competitive employment outcome. The program manager will coordinate care in a client-centered manner, focusing on individual strengths, and the needs of the client, and conducting vocational evaluations to assist the client in job placement. Will work with other service providers to ensure wrap around services are provided and with future employers and other outside partner agencies.
