Ashby Family LEAD Engagement Services
Position: Direct Care Professional
Direct Care Professional requirements:
⢠Coach, teach, and mentor people with intellectual disabilities
⢠Assist and train clients with all areas of daily living skills
⢠Implement Individual Support Plan programs, plans, and services
⢠Transport and accompany clients to appointments, or other activities in the community
Skills and Abilities
⢠Outgoing and friendly
⢠Ability to provide accurate and clear written communications both electronically and verbally
⢠Punctual and responsible
⢠Ability to work with individuals with visible and invisible disabilities
⢠Effectively communicate with management, peers, residents, and the public as needed
⢠Able to work in a team and alone
⢠Able to read and follow policies and procedures
⢠Maintain great work habits.
Education & Experience
High School Diploma or GED required. Experience working with individuals with disabilities, preferred. A comparable amount of training and experience may be substituted for the minimum qualifications.

Position: Community Inclusion Specialist
Overview: The Direct Care Professional plays a key role in supporting individuals with diverse abilities to actively participate and engage in community activities. This position involves providing direct care, fostering social connections, and promoting community inclusion for program participants.
Responsibilities:
1. Individual Support:
⢠Providing direct care and support to individuals with diverse abilities in community settings.
⢠Fostering independence and assisting with activities of daily living as needed.
2. Community Integration:
⢠Planning and implementing community-based activities to enhance social inclusion.
⢠Facilitating the participation in recreational, educational, and vocational activities.
3. Goal Setting:
⢠Collaborating with the program participants to set personal goals related to community inclusion.
⢠Supporting individuals in achieving their goals through encouragement and guidance.
4. Communication:
⢠Maintaining an open and effective communication with program participants, their families, and support teams.
⢠Acting as an advocate for individuals to ensure their needs and preferences are understood and respected.
5. Documentation:
⢠Keeping an accurate and up-to-date records of participant progress, activities, and any incidents.
⢠Preparing reports as required by organizational policies and regulatory standards.
Qualifications:
⢠Experience working with individuals with visible and invisible disabilities
⢠High school diploma or GED.
⢠Valid driver's license, reliable vehicle and able to provide current insurance
⢠Excellent interpersonal skills, a caring approach & physically fit
⢠National Caregiver certification preferred
⢠Candidates must successfully pass a background check and complete fingerprinting
⢠Experience working with underserved marginalized communities
