Job Details FLSA Status: Non-Exempt Job Type: Full-time Full-time (24 hrs direct care + 16 hrs indirect/administration weekly) Location(s): Imperial Base Compensation: $58,000 - $62,000 /year, commensurate with experience Designated Coordinator Supervises: Direct Support Professionals Location: Crystal Supervisory Responsibilities: Oversee scheduling, timecard approval and 245D required paperwork Directly supervise and lead a team of support staff and direct care professionals. Provide guidance, training, and performance feedback to ensure the team delivers high-quality care and support. Responsibilities: With the team, including legal guardians, develop and implement individualized care plans appropriate for the individual Oversee any support strategies to assist the individual with gaining and maintaining skills, health and wellbeing Manage the overall daily schedule and activities to support preferences, health and wellbeing, community engagement and skill development Manage, coordinate and supplement scheduling to meet the individuals and company needs Maintain a team environment consistent with the vision and agreed upon expectations of the location Train new staff utilizing adult education principles, ensuring they are well-equipped to meet the needs of the individual and in compliance with all 245D training requirements. Plan and conduct regular staff meetings and trainings to discuss program updates, individual progress and address any concerns or challenges. Provide ongoing training opportunities to enhance their own and other staff members skills and knowledge Ensure the safety of the individual by ensuring health and safety maintenance plans in place are executed consistently and regularly, including food safety, falls prevention, proper nutrition and daily hygiene. Maintain accurate and up-to-date records, including incident reports, medical and medication records, and individual progress reports Report any health red-flags to guardians, following established health protocols if illness arises Act as a liaison alongside the individual with their families, external service providers and the organization, fostering and maintaining effective communication Advocate for the rights and choices of the individual, promoting self-advocacy whenever possible Attend meetings and collaborate with interdisciplinary teams to coordinate services and support for the individual Manage programs that meet individualized Home Supports with Training guidelines. Skills Required Able to communicate effectively while managing people Experience working with individuals with developmental disabilities and who are non-verbal Experience managing a household, including menu planning, cooking, cleaning and budgeting Able to problem solve with others on the spot, including taking texts or phone calls when not with the individual Active, non-sedentary lifestyle including the ability to ride a bike and swim First Aid certified Able to work evenings and weekends as needed Ability to be playful, have fun and utilize a sense of humor Qualifications In order to qualify as the Designated Coordinator as defined in Minnesota Statute Chapter 245D.081, you must meet one of the below requirements: A baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older. An associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older A diploma in a field related to human services, and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older. A minimum of 50 hours of education and training related to human services and disabilities; and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the requirements identified in (1) to (3) above. Additional desired qualifications include Previous experience in a supervisory or management role within the field of developmental disabilities Strong understanding of person-centered planning and support methodologies. Proficiency in relevant computer applications for record-keeping, reporting and timecard approval/EVV. Physical Requirements: Ability to lift and carry objects up to 50 pounds. Stamina and physical agility to respond to emergencies and aid when needed. Mobility to navigate the individual’s home and accompany him in the community as he lives his best life Visual and auditory acuity to be able to monitor the well-being and safety of the individual, the staff and themselves. Ability to manage universal sanitary procedures and provide dignified personal cares such as toileting, showering, and any and all personal cares needed
