What you’ll Do:
Under the direction of the designated Supervising Social Worker, this position using Person Centered practices assesses the needs of developmentally disabled individuals and their families and develops and implements the person’s Individual Program Plan and coordinates associated services. This position has the responsibility for managing a wide range of active cases requiring the skills of a professionally trained social worker, including creative planning in seeking and acquiring services for individuals supported.
This position meets with the individuals at their residence or in the community to design and develop the Individual Program Plan for the person supported, then implements, monitors and revises the program plan, assess needs and identifies barriers to individual’s developmental objectives. Initiates and terminates regional center funding and insures that purchased services are provided. Prepares required reports and documentation in a timely and accurate manner. Maintains records in compliance with federal and state requirements.
Acts as an advocate with individual supported and their family to assure receipt of entitled services and to assure client's rights are achieved. Encourages individuals supported and their families to fully participate in coordinating and utilizing the resources available and reviews and insures that the individuals supported receive appropriate and effective services from existing community resources.
Initiates referrals and monitors progress of vendored services, evaluating availability and effectiveness of services. Identifies community service needs or gaps and assists community resources in the development of needed services and improving the quality of services.
May provide intake services and outreach services as assigned. Participates in internal and community committees and task forces as assigned.
Requirements
You have;
Knowledge of principles, objective and techniques of counseling individuals;
Knowledge of normal and abnormal individual and group behavior, human growth and development of family relationships;
Knowledge of community structure and organization and how to intervene constructively on behalf of individuals supported;
Knowledge in the areas of: identifying and navigating generic services, objective and techniques of counseling individuals and typical and atypical child development behavior;
Ability to interview skillfully;

GGRC is currently hiring for a Physician to provide medical evaluations as part of the assessment process to determine eligibility for potential clients and provide consultation on medical matters to Social Workers and other staff in their on-going efforts to serve clients. This role will be expected to work 32 hours a week and offers a hybrid schedule offering the opportunity to work out of any of our three offices in San Mateo, San Francisco and San Rafael, CA. This role will also require you to travel within the Counties of San Francisco, San Mateo and Marin. Occasional statewide travel to sites throughout California. Access to reliable transportation permitting travel to and between many different locations throughout the day to perform the outlined responsibilities of the position.
What You'll Do:
Client Assessment & Support
Assessment of individuals for eligibility purposes and level of function
Determine medical diagnoses.
Contact primary physicians and other agencies to obtain medical/health information and clarify or otherwise discuss diagnoses, treatment and other pertinent concerns.
Arrange for specialty examinations and tests.
Reviews records pertaining to clients’ medical and developmental history.
Serves as lead member of Interdisciplinary Team in determining eligibility for center services.
Participate with other GGRC professionals in planning for appropriate client care.
Develop a plan to ensure that clients receive appropriate medical care.
Counsels others as appropriate, such as clients’ families, on the medical aspects of developmental disability and care needs.
Serve as consultant on complex medical issues for continuing cases, including the use of psychotropic medications & poly pharmacy.
Coordinate and serve as a liaison for family and case manager with medical/psychiatric/dental and other providers of care as necessary.
Work closely with nurses and case management staff on the clinical team for issues related to Consents for medical treatment/Advance directives.
Organizational Support
Coach case managers on signs relating to medical developmental and behavior characteristics of the children we assess for eligibility.
Serve as consultant for other regional center professionals.
Serve on regional center and community committees as needed.
Function as a liaison with the general medical community.
Advocate for prevention services.
Participate in in-service training programs.
Represents the center in the community and in medical associations.
Train local pediatric interns and residents about the Regional Center system and Developmental Disabilities.

HOURS + LOCATION

Evenings: 1–2 evenings per week, concluding at 8:00 PM

Must reside within the State of California.
Board Certified/Eligible in designated specialty field of practice is required.
Must be in possession of a current medical license to practice in the State of California, including DEA license, and CPR.
Previous experience providing medical care within a community health setting preferred.

Essential Functions
Responsible for nursing care, medication administration, and treatments for the assisted living residents in compliance with Federal and State regulations and the Masonic Homes guidelines.
Responsible for the admissions of new residents to include all admission assessments, the assessment of resident health status for both new and existing residents, the subsequent development of the service plan, and documentation.
Recognize changes in conditions of residents and notifies physicians and family members of any change in resident's health, and provides proper documentation.

Initiates writing and updating Resident Assessment and Service Plans as resident's condition changes, collaborating with active living, social services, and dietary services.

LVN maintains close communication with all departments and staff members, Health Services Director, health care professionals, and families about the needs of residents.
Notifies MD of resident changes and concerns after consultation with RN;
Personally receive or place calls to physician, receives and processes MD orders, ad orders medications as needed.
Monitors medications of residents as ordered by MD;
Administer intramuscular, intradermal, and subcutaneous injections.
Chart medications and treatments according to the procedure.
Verify inventory of drugs covered by the Controlled Substances Act of 1970.
Ensure that individual resident assessment and service plan are followed.
Attends family conferences;
Understands role in the Safety and disaster plan.
