The facilitator will provide service recipients high-quality community integration training, volunteer opportunities, and employment development and exploration. Conducts client contacts, schedules, and weekly activities consistent with the client's Individual Service Plan (ISP) long- and short-term goals. A facilitator will also assist the client and other planning team members with the intake and assessment process if the Program requires it. Depending on the physical needs of clients, some self-care assistance, such as toileting and feeding, may be necessary to support the client's success. Client Facilitator reports to assistant director. Client Facilitator supervises no other staff in the facility. Minimum qualifications: Must be at least 18 years of age. Must Pass Health Screening and TB test. Must be first Aid and CPR certified. Must pass Criminal Record Clearance (DOJ and FBI Background check) High school diploma or at least one year of experience working with individuals with developmental disabilities. Experience is essential, but motivated & enthusiastic individuals, who lack experience in the field, but are eager to work in a rewarding social services area, will receive the necessary orientation. Applicant must have reliable transportation and proof of insurance. Throughout employment. Must have a valid California Driver's license, have 24 months of driving experience. Valid CA Driver's License with a clean driving record Immediate access to the vehicle with working seatbelts to transport participants Are knowledgeable of SDRC referral and funding procedures.
