We have a live-in aide/ Companion / Caregiver position available for a thoughtful, reliable person with great people skills. A lot of our clients used to live in group homes or institutions, and now enjoy the freedom to live as they choose. CURRENT CLIENT OPENINGS: This position is located in Bay Village in Watsonville close to East Lake shopping center. We currently only have one 24 hour shift available per week with this couple. The candidate that is chosen for this position must be available for and will be able to choose from one of the following shifts: Wednesday 3p-Thursday 3p -or- Thursday 8a- Friday 8a. Important to note, this is a live-in position. The candidate who is chosen for this position will be able to live in/occupy the second bedroom of the home 24/7, even when they are not on shift. Opportunities for shift coverage and work in other homes are available for those who may be interested in more hours. This is a great opportunity for a person who is looking for paid work as well as housing. In this position you would be serving a developmentally disabled couple who also happen to be senior citizens. They need help with cooking, cleaning, basic hygiene, medication administration etc. They are both ambulatory so they navigate their home and the community well and are both pretty independent. They mostly sleep through the night (8-10 hours usually) with the exception of a 1am bathroom reminder. Folks who have a car for an outing would be appreciated but it is not required. At 24 hours per week this is a part time position and would therefore not qualify for benefits- medical, dental and life insurance. For those interested in benefits we can build up to a full time schedule. COMPENSATION: The pay is $20.50 per hour for awake hours (8a-midnight), paid by IHSS through the county of Santa Cruz so you'll need to enroll in In-Home Support Services to become a provider. For overnight shifts, you're paid $16.50 per hour to sleep during sleep hours, up to 8 hours a night (midnight-8a). Your IHSS registration expenses will be promptly reimbursed. ESSENTIAL DUTIES AND RESPONSIBILITIES: • Provide clinical support to medical providers before and after primary care visits so as to ensure the timely delivery of quality medical care. • Interface with medical assistants to ensure patients receive specialty referrals in a timely manner and are able to follow through so as not to interrupt patient care. • Treat patients diagnosed with sexually transmitted infections (STIs) in a timely manner and according to APLAHW clinical STI treatment guidelines. • Administer vaccines to patients per APLAHW clinical guidelines. • Provide and appropriately document health education to patients. • Participate in quality improvement activities as directed by the medical director, e.g. data analysis and measurement of outcomes, document and report the results and accomplishments of quality improvement initiatives. • Triage patients who walk in to clinic or call with acute medical concerns. • Assist front office staff in making appropriate arrangements for patients who arrive late for appointments or for whom an appointment needs to be rescheduled, by reviewing the medical record and determining how soon and where the patient should be seen. • Provide guidance for the clinic LVN and MAs by overseeing the front and back office staff with the aim of maintaining efficiency, professional demeanor and courtesy toward patients. This may include serving as back up for the front and back office when needed. • Assist in the clinic transformation to a patient centered medical home (PCMH), in conjunction with the PCMH coordinator, medical director, chief operating office and clinical nurse manager. • Maintain adequate medical supplies and medications for clinical areas by monitoring supplies and ordering supplies as needed. Ensure that exam rooms are fully stocked and prepared for patient care. • Participate in chronic disease management for selected patients with special needs. • Communicate lab results to patients. • Assist patients in linkage to APLA services: benefits counseling, mental health, case management, dental etc. • Serve as first filter to eCW patient portal, answering when appropriate within his or her scope. While based at our Gleicher/Chen clinic, the Medical Care Manager also supports patients at our Mid-Wilshire and Michael Gottlieb (West Hollywood) Heath Centers. The RN medical care manager will coordinate with all parties involved in the care of the patient to provide effective and culturally competent care and treatment for people living with HIV and other chronic diseases. The position provides services to persons living with HIV as part of a Medical Care Coordination team and providing case management services for persons with other complex medical needs. The medical care coordination team will target people with HIV who are experiencing medical adherence issues, significant changes in HIV health status or multiple health diagnoses that affect the person’s HIV status. The RN medical care manager in conjunction with the Patient Care Manager, formulates care plans based upon assessment data and provision of care priorities and works in collaboration with the clinical social worker as necessary. The RN medical care manager will coordinate patient care and collaborate with other health care team members to establish the patient’s goals, develop treatment plans and obtain desired outcomes as well as provide patient education based on identified learning needs utilizing available teaching resources. ESSENTIAL DUTIES AND RESPONSIBILITIES: • Provides clinical support and advocacy to HIV+ patients, such as support in improving medication adherence and risk reduction. • Identifies out of care HIV+ patients to reengage them in treatment • Performs periodic needs assessments for patients to monitor and update medical needs and to intervene appropriately to support better health outcomes. • Identifies patients’ unmet medical needs and coordinates with clinic HIV and non-HIV providers to ensure that standards of care are met and any gaps or barriers are addressed. • Assists in tracking, following up with, and supporting existing and recently enrolled patients • Identifies patients’ unmet nonmedical needs or barriers to adherence and retention in care, and coordinates with other clinical staff (e.g. social workers) to address needs and barriers. • Provides and appropriately documents brief interventions focused on patient education, treatment adherence, managing side effects, medical nutrition therapy, co-infections, preventative care and risk reduction. • Participates in quality improvement activities as directed by the medical director, e.g. data analysis and measurement of outcomes, document and report the results and accomplishments of quality improvement initiatives. • Assists front office staff in making appropriate arrangements for patients who arrive late for appointments or for whom an appointment needs to be rescheduled, by reviewing the medical record and determining how soon and where the patient should be seen • Provides health supervision to persons living with HIV and non-HIV on a continuing basis under the consultative direction of a physician OTHER DUTIES MAY BE ASSIGNED TO MEET BUSINESS NEEDS.
